Requirements
  • A Registered Nurse license in good standing is required.
  • A minimum of 2 years of related nursing experience as a Registered Nurse or Licensed Practical Nurse is required.
  • One to three years of supervisory experience is required.
  • Knowledge of state and federal laws and regulations governing skilled nursing facility operations, including Colorado requirements, is required.
  • The candidate must understand long-term care and hospitals and the importance of technology in improving resident care.
  • Computer word-processing and database experience is required.
  • The candidate must be able to read, write, and speak English.
  • The candidate must be able to handle objects weighing up to 20 pounds occasionally and/or up to 10 pounds frequently and must sit, stand, bend, and move intermittently during working hours.
Responsibilities
  • Coordinate the work of team members to meet resident goals and expectations.
  • Perform post-admission screening to determine residents' current knowledge of disease processes.
  • Assess residents' baseline activation level using the PAM assessment and coach them to improve their scores.
  • Develop coaching relationships with residents and relevant stakeholders and empower residents to participate actively in their plans of care.
  • Follow up with communication, support, and education for residents, families, and team members to reduce post-discharge readmission risk.
  • Coordinate and evaluate care conferences, care-plan updates, and Med A meetings, including discharge plans and rehabilitation and transition goals.
  • Oversee discharge planning and case management.
  • Implement resident education materials and disease-management teaching.
  • Ensure effective internal and external communication to foster continuity of care.
  • Evaluate resident readmissions and make recommendations for QAPI and quality-improvement practices.
  • Implement INTERACT tools with staff to promote improved care practices, communication, and reporting.
  • Identify residents who need advance care planning and initiate palliative or hospice care.
  • Review quality-indicator reports and implement corrective action, including QAPI for hospital readmissions.
  • Assist with clinical oversight as required.
  • Support and integrate Masterpiece Living and Eden Alternative initiatives that promote successful aging, person-centered programs, and culture.
  • Evaluate the need for, organize, and coordinate educational programs in accordance with federal, state, and local regulations and company policies.
  • Monitor care delivered by nursing staff and implement corrective action as necessary.
  • Manage rehabilitation services, including vendor relationships and rehabilitation competency and training programs.
  • Assist the Director in managing assigned team members through coaching, training, corrective action planning, goal setting, applicant interviews, and performance appraisals.
  • Act as Director of Nursing in the manager's absence.
Desired Qualifications
  • A Bachelor of Science in Nursing is preferred.
  • Leadership abilities and comprehensive knowledge of healthcare transitions, including PPS, MDS, OBRA, and OSHA, are preferred.
  • Administrative experience is preferred.
  • Excellent customer service skills are preferred.

Ziegler closes $41.2M financing for Christian Living Communities in Colorado

Ziegler has closed $41.2 million in Series 2026 Bonds for Christian Living Communities Obligated Group through the Colorado Health Facilities Authority. The organisation operates three senior living communities in Denver metro comprising 474 independent living units, 122 assisted living apartments and 192 skilled nursing beds. Proceeds will refund Series 2016 Bonds and fund $2 million in capital expenditures, generating over $4 million in debt service savings. Fitch awarded the Obligated Group a "BBB+" rating with stable outlook in connection with the issuance. Christian Living Communities' parent organisation includes three additional assisted living facilities, a home care agency, a Medicare Advantage programme and third-party management company Capella Living Solutions. Ziegler is the leading underwriter of financings for not-for-profit senior living providers in the US.
