Requirements
  • A Master’s degree in Social Work (MSW)
  • Minimum of one year related experience in a hospital environment and/or with transferable skills that apply to a fast-paced and high-urgency setting
  • Ability to triage crises and collaborate/coordinate with collateral agencies
  • Experience with protective services, child welfare, long term care, crisis intervention, chronic illness, hospice care, substance use disorders and psychiatric illnesses (a plus)
  • Licensure requirements can be discussed and obtained with an agreed upon timeline
  • Schedule requires 40 hours day shift; weekend and holiday coverage will be required
  • All staff must be vaccinated against influenza as a condition of employment
Responsibilities
  • Perform psychosocial assessments and provide supportive counseling
  • Coordinate care and discharge planning and service coordination for adult, geriatric, pediatric, and medical/surgical populations
  • Collaborate with the care coordination team including RN case management, clinical team, and collateral agencies
  • Engage in interdisciplinary team activities to support patient-centered care
  • Assist with care coordination duties across various services to support patient outcomes

  • Weekly clinical supervision for Licensed Independent Social Worker candidates
  • New graduates are encouraged to apply
  • Experience with pediatric and geriatric populations and complex social work cases in a hospital setting is a plus
