- Board certification through AANPCB or ANCC in family, adult, geriatric, or adult-gerontology practice is required.
- An active Drug Enforcement Administration license and controlled-substance registration are required as applicable.
- At least 3 years of clinical experience as a Nurse Practitioner is required.
- Basic Life Support certification is required.
- A valid driver's license and reliable transportation are required.
- Experience with electronic health record documentation is required.
- Commitment to equitable, patient-centered care is required.
- Provide ongoing primary and preventive care to older adult patient populations.
- Manage and grow a dedicated patient panel within assigned communities.

- Use telehealth and in-person care delivery models to improve accessibility.

- Experience in value-based care settings is preferred.
- Experience providing care in community-based, in-home, or non-traditional healthcare environments is preferred.
- Familiarity with MEAT documentation and chronic-condition coding is preferred.
- Telehealth experience is preferred.
- Experience with athenahealth is preferred.
