Requirements
  • Graduate of an accredited school of nursing or have an education equivalency for licensing.
  • No experience is required.
  • Provide proof of motor vehicle liability insurance meeting minimum state requirements upon hire and upon request.
  • Hold Basic Life Support HealthCare Provider certification from the American Heart Association.
  • Hold the applicable registered nurse license for the state of work location, including a compact-state, Missouri, Illinois, Oklahoma, or Wisconsin registered nurse license as applicable.
  • Have a satisfactory motor vehicle report and a state-issued driver license.
  • Meet the physical requirements of the role, including frequent driving, standing, walking, lifting, keyboard use, and occasional patient movement.
Responsibilities
  • Coordinate, facilitate, and perform an interdisciplinary plan of care by assessing and evaluating patient condition and care effectiveness under the direction of the attending physician.
  • Travel to patient homes, community-based residential facilities, assisted living locations, hospitals, and other settings to meet patients and their families or caregivers and assess care needs and acuity.
  • Assess and coordinate patients’ discharge-planning needs with members of the healthcare team.
  • Perform point-of-care testing according to policies and procedures when required.
  • Collaborate, consult, and communicate with physicians regarding plans of care and changes in patient status; provide progress updates, recommend plan revisions, and document communications in accordance with regulations.
  • Coordinate and communicate with multidisciplinary team members and outside agencies to achieve patient-care and education outcomes through end of life or discharge.
  • Complete admission paperwork, verify insurance information, and enter comprehensive medical assessment data into the clinical computer program.
  • Provide skilled nursing care within the established plan of care, including taking vital signs, wound care, intravenous care, ostomy care, blood draws, injections, and physical assessments of patients’ medical conditions.
  • Document visit notes, treatments, and care; follow physicians’ visit orders and ensure required visits are completed accurately and on time.
  • Collaborate with patients, families, and the healthcare team to develop comprehensive, individualized plans of care.
  • Respond to calls from patients, family members, and others, and follow up to address needs, issues, or concerns.
  • Work in a constant state of alertness and in a safe manner.
  • Perform other assigned duties.
