Requirements
  • Graduate of an accredited nursing program.
  • Holds a current State of Idaho registered nurse license.
  • Minimum 3 years of experience in a clinical setting.
  • Ability to communicate effectively, both orally and in writing.
  • Skill in telephone-based clinical assessment.
  • Ability to maintain a work pace appropriate to the workload.
  • Ability to multitask, prioritize, and organize daily responsibilities.
  • Exceptional listening skills and the ability to elicit information needed to make a brief evaluation.

Responsibilities
  • Provide medical information and symptom triage to assess the severity of patients’ health concerns using established medical guidelines that support the call center process.
  • Communicate information, make recommendations, and verify patient understanding regarding healthcare issues and concerns.
  • Evaluate and assess patients’ healthcare needs based on signs and symptoms using established triage protocols and guidelines.
  • Obtain appropriate input and direction from the provider as needed to determine the best clinical course of action and recommendations for specific patient healthcare needs.
  • Accurately document symptoms or complaints, nursing assessments, advice provided, and patient or caller responses.
  • Follow policies, procedures, and protocols to ensure consistency and departmental effectiveness and improve patient and caller healthcare outcomes and access to appropriate healthcare.
  • Coordinate and schedule same-day appointments on providers’ schedules.
  • Manage prescription refills according to protocol.
  • Demonstrate a commitment to patient-centered care.
  • Participate in assigned performance-improvement activities.
  • Help establish workflows and processes.
  • Assist with patient scheduling during downtime.
  • Participate as a member of the Heritage Health Patient Centered Medical Home Care Team.
  • Perform miscellaneous job-related duties as assigned.
