Requirements

Responsibilities
  • Verify that sufficient information is available for accurate insurance verification and eligibility, contacting the physician office or patient when necessary.
  • Determine whether secondary insurance should be added to the patient account and select the appropriate primary payer.
  • Use the center's selected claims and eligibility vendor or individual payer websites to obtain eligibility, benefits, precertifications, and authorization information.
  • Enter patient insurance information into the patient accounting system with the appropriate payer and financial class.
  • Notify the manager immediately when a patient's insurance is out of network and follow policies and procedures for accepting out-of-network payers.
  • Document insurance verification findings in the patient account and contact patients with estimated coinsurance, copayment, or deductible amounts due on or before the date of service, as applicable.
  • Contact each patient before the date of service to review physician ownership, advance directives, and patient rights in accordance with CMS Conditions for Coverage guidelines.
  • Obtain authorizations from insurance companies or physician offices and ensure the authorization contains complete and accurate information, including the procedure, date of service, facility name, and expiration status.
  • Enter authorizations into the patient accounting system, including the name, Current Procedural Terminology codes, effective date, and authorized procedures.
  • Ensure high-cost implants, supplies, or equipment rentals are included on authorizations.
  • Check insurance company approved procedure lists and medical policies, notify patients when procedures are not payable, and obtain a signed Medicare Advance Beneficiary Notice or other non-covered notification form when patients choose to proceed.
  • Calculate patient copayments and estimated coinsurance according to payer contracts and plans.
  • Obtain preapproval from the SCA Compliance Department when accepting in-network benefits for out-of-network payers.
  • Apply payer guidelines and collect over-the-counter payments, deductibles, copayments, and coinsurance.
  • Contact patients and communicate the center's financial policy.
