Requirements
  • An Associate’s Degree in Nursing is required; a nursing diploma is acceptable, and a Bachelor’s Degree in Nursing is preferred.
  • Registered Nurse licensure in Rhode Island is required for CCBHC opportunities.
  • A Rhode Island Licensed Practical Nurse license is required for specific opportunities.
  • Experience working with psychiatric or mental health patients and in substance use treatment is preferred.
  • A valid driver’s license, vehicle registration, current inspection sticker, and insured automobile are required.
  • Knowledge of individuals with mental health issues, their families, and family systems is required.
  • Skill in crisis assessment, risk management, and de-escalation is required.
  • Ability to remain professional and calm during emergencies is required.
  • Ability to perform arithmetic calculations using whole numbers, common fractions, and decimals is required.
  • Ability to compute rates, ratios, and percentages and interpret bar graphs is required.
  • Knowledge of community resources is required.
  • Proficiency in documentation and electronic record-keeping systems, including electronic medical records, is required.

Responsibilities
  • Perform initial, annual, and ongoing nursing assessments and update clients’ care plans.
  • Provide services to an assigned caseload and other clients as needed, meeting program service-intensity standards through office- and community-based contacts.
  • Triage clients and conduct comprehensive health, psychiatric, substance use, housing, and safety assessments.
  • Identify, assess, and respond to crises through risk management and crisis stabilization.
  • Develop, implement, and coordinate individualized care plans, including client goal setting.
  • Coordinate hospitalizations and post-discharge follow-up during care transitions.
  • Provide integrated care aligned with medical protocols and question care plans when safety or wellness is at risk.
  • Document assessments, laboratory results, medication updates, and required records according to agency standards.
  • Administer medications, document them accurately in the electronic medical record, and complete medication reconciliations.
  • Implement prescribed medication regimens according to the six rights of medication administration.
  • Educate clients and families about diagnoses, care plans, rehabilitation, and medication effects and side effects.
  • Uphold infection prevention, patient safety, and regulatory standards and report concerns according to agency policies.
  • Attend team meetings and case reviews to share updates and address client challenges.
  • Conduct home visits, school meetings, and community outreach.
  • Assist clients with daily activities and life skills, including basic medical procedures, hygiene, medication management, and budgeting as appropriate.
  • Maintain accurate, detailed, and timely records of client interactions and services.
  • Ensure compliance with agency policies, ethical standards, and regulatory requirements.
  • Transport clients safely to appointments and activities as needed.
  • Maintain cooperative relationships with staff, clients, and community partners.
  • Attend trainings and case presentations and complete professional development activities needed to maintain valid licensure.
  • Participate in training and orientation of new staff and student nurses as needed.
  • Serve on committees and participate in agency initiatives as assigned.
  • Perform other duties assigned to support client goals and program needs.
Desired Qualifications

  • A Bachelor’s Degree in Nursing.
  • Bilingual proficiency; for positions designated as bilingual, pass the required language assessment within 30 days of starting.

Care New England cuts 30+ leadership jobs to close $20M budget gap

Care New England is cutting more than 30 leadership and non-clinical positions to address a projected $20 million budget deficit for fiscal year 2026. The announcement comes as hospitals nationwide face financial pressures from federal Medicaid funding changes and policy overhauls. CEO Dr Michael Wagner cited uncompetitive state reimbursement rates compared with neighbouring states as factors hindering staff recruitment and retention. The health system, Rhode Island's second-largest hospital operator, employs over 8,000 people across Butler, Kent and Women & Infants hospitals. Care New England has faced ongoing financial challenges, exacerbated during the pandemic. A proposed 2022 merger with competitor Lifespan was blocked by regulators on anti-trust grounds. The company spent more than $10.8 million on executive compensation in fiscal 2024.
