Responsibilities
  • Conduct outbound calls (est. 50/day) and digital outreach to patients, informing and educating how Colla Health’s platform serves as an extension to a partner practice’s care team
  • Achieve member enrollment goals as driven by company KPIs
  • Manage inbound calls from potential patients
  • Support patients by answering their questions, addressing concerns and providing guidance on how Colla Health can support them
  • Document all interactions and maintain timely, accurate records of patient outreach activities
  • Collaborate internally and externally with healthcare providers, community organizations, and other stakeholders to ensure patients receive appropriate care and services
