Requirements
  • Bachelor's degree from an accredited college or university in social work, social services, human services or a related field.
  • Two years of full-time, paid, professional experience providing case management to children and families in an urban environment.
  • All work experience related to the position must be included on the resume.
Responsibilities
  • Engages with the family and assists with identifying resources to enhance treatment; works collaboratively with STEP to assist with obtaining necessary outside supports for students.
  • Oversees comprehensive educational support services to address the social, emotional and social service needs of students and families by addressing unmet physical, living situation and behavioral health needs to help stabilize the child and family.
  • Provides case management services to students and families to promote and support students' physical and emotional wellness to bolster academic and social success; develops a plan of action and implements it to successful completion.
  • Provides referral services and access to resources to support students and families; serves as the link between the home, school and community.
  • Maintains and updates client case files and records by reviewing case notes, logging events and tracking progress.
  • Assists in the development and implementation of appropriate programmatic and administrative policies; ensures compliance with STEP and School District of Philadelphia policies, and Medicaid regulations; follows all state Medicaid guidelines as it pertains to service quality and documentation.
  • Makes home visits to encourage parents to provide support for students.
