Requirements
  • Bilingual Spanish proficiency (spoken and written) required.
  • Graduate of an accredited school of nursing.
  • Certified as a nurse practitioner with current state licensure.
  • At least 1 year of previous professional nursing experience with 1-3 years experience as a practicing as a primary care nurse practitioner.
  • Prior experience working in an electronic medical records system.
  • Provide 5 Star service in accordance with the Better Health Group's mission.
  • Skilled in traditional nursing disciplines of patient assessment and management.
  • ACLS/BLS certification required as mandated by state.
  • Must demonstrate knowledge of the rationale of appropriate patient care.
  • Demonstrate ability to handle emergency situations calmly and effectively.
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to effectively present information and respond to questions from a various group of people.
  • Ability to define problems, collect data, establish facts, and draw valid conclusions.
  • Excellent written and verbal communication, teaching ability, and good telephone skills.
  • Working knowledge with Google Suite (Drive, Docs, Sheets, Slides) and Microsoft Office (Word, Excel, PowerPoint) for real-time collaboration.
Responsibilities
  • Practice medicine under the supervision of a physician using sound medical judgment and adhering to all ethical considerations of the practice of medicine; knowing limitations and seeking consult when advisable; keeping abreast of current technologies and industry standards
  • Under the supervision of a physician, provide care for patients with acute and chronic illness encountered in the older adult patient
  • Under the supervision of a physician, accountable for patient care and outcomes and will appropriately seek consultation from specialists when needed, though will stay involved in, and be responsible for, the detailed care of the patient
  • Engage with hospitalists whenever a patient is in the hospital
  • Meet all clinical obligations of the medical practice: Accrediting Agency compliance, CLIA compliance, drug prescription compliance, HIPAA, State and Federal regulations, Safety Regulations, etc
  • Assess, diagnose, treat, manage, educate, promote health, and provide care coordination and documentation for patients with acute and complex chronic health needs using evidence-based medicine and practice guidelines
  • Follow all internal policies and procedures
  • Accountable for the completion of all operational tasks according to departmental guidelines to ensure appropriate clinical processing/documentation
  • Maintain complete and detailed records, electronically in the EMR, of patients’ health care plans and prognosis, completed within 48 hours of each patient’s visit
  • Provide patients with information needed to promote health, reduce risk factors, or prevent/manage disease or disability
  • Analyze and interpret patients’ histories, symptoms, physical findings, or diagnostic information to develop appropriate diagnosis
  • Interpret data and diagnostic results for an appropriate course of action including but not limited to blood gases, radiology, EKG, etc. in collaboration with supervising physician
  • Exhibit behavior that emphasizes compassion, quality, loyalty, and conscientiousness
  • Ability to establish and maintain effective working relationships with patients, their families, and team members
  • Document continuing education per licensure and certification requirements
  • Ensure that patients are seen every other visit by the Physician
  • Maintain patient confidentiality in accordance to policy and procedure and HIPAA requirements
  • Enact proper chain of command for patient complaints
  • Additional duties as assigned
