Requirements
  • Registration with the College of Licensed Practical Nurses of Alberta as a Licensed Practical Nurse with active status.
  • A valid driver’s license and access to a vehicle are required.
  • Willingness to travel to and from patient residences within a designated area.
  • Required on-call coverage, including after-hours coverage, to ensure patient safety and meet patient care needs.
Responsibilities
  • Participate in scheduled community visits to address patient needs within the scope of practice.
  • Provide ethical and safe nursing care.
  • Use appropriate assessment tools and techniques to assess patients’ individual needs.
  • Execute care plans, continuously monitor them, and adjust them as required.
  • Engage patients through a collaborative approach by building rapport and trust.
  • Maintain and update professional patient records related to patient encounters and care plans.
