Requirements
  • A Nursing Degree from an accredited college or university, or graduation from an approved Licensed Practical Nurse/Licensed Vocational Nurse program is required.
  • A current, unencumbered, active license to practice as a Licensed Practical Nurse/Licensed Vocational Nurse in the state is required.

Responsibilities
  • Provide direct nursing care to residents and supervise the day-to-day nursing activities performed by nursing assistants.
  • Direct the day-to-day functions of nursing assistants and ensure nursing personnel comply with facility policies, procedures, job descriptions, and applicable regulations.
  • Review department policies, procedure manuals, and job descriptions and recommend revisions.
  • Plan shift services, programs, and activities with assigned nursing and support personnel.
  • Prepare written and oral reports and recommendations concerning shift activities.
  • Coordinate nursing services with other resident services to maintain each resident's total care regimen.
  • Participate in the nursing service quality assurance program and facility surveys conducted by authorized government agencies.
  • Review and update resident discharge plans and assist with planning nursing services for discharge.
  • Interpret department policies and procedures for personnel, residents, visitors, and government agencies.
  • Admit, transfer, and discharge residents and complete related forms, reports, census records, charge slips, and medical documentation.
  • Protect resident and facility information by maintaining password confidentiality, protecting protected health information, and reporting unauthorized access attempts.
  • Complete admission, transfer, discharge, accident, incident, and routine charting documentation.
  • Receive telephone orders from physicians and transcribe physician orders to resident charts, medication cards, treatment plans, and care plans.
  • Chart nurses' notes descriptively, document resident responses to care, and report discrepancies in orders, diets, or charting.
  • Prepare and administer physician-ordered medications and treatments after verifying resident identity.
  • Maintain medication, narcotic, floor stock, supply, and equipment records and report discrepancies or shortages.
  • Review medication cards, notify physicians about automatic stop orders, and dispose of drugs and narcotics according to procedure.
  • Participate in employee performance evaluations, determine shift staffing needs, and recommend personnel actions to the Nurse Supervisor.
  • Report staffing shortages and absentee call-ins and develop or assist with work assignments.
  • Provide leadership to assigned nursing personnel and make daily rounds to verify that work assignments meet nursing standards.
  • Ensure nursing assistants are enrolled in or have graduated from approved training programs and that nurse aide trainees are directly supervised by a licensed nurse.
  • Meet regularly with nursing personnel to identify and correct problems and improve services.
  • Receive and give nursing reports at the beginning and end of shifts.
  • Report occupational exposures, suspected fraud, incidents, and other safety or compliance concerns.
  • Inform nursing personnel about admissions, prepare rooms, greet and orient new residents and family members, and make rounds with physicians as needed.
  • Arrange diagnostic, therapeutic, transportation, and discharge services as ordered or required.
  • Consult with physicians regarding resident care, treatment, and rehabilitation and notify physicians and next of kin about accidents or changes in resident condition.
  • Review charts, medications, diets, care plans, and treatments and ensure that prescribed care is properly administered.
  • Carry out restorative and rehabilitative programs and provide professional services including catheterization, tube feedings, suction, dressing changes, colostomy and drainage-bag care, specimen collection, vital-sign measurement, and range-of-motion exercises.
  • Monitor seriously ill residents, inspect treatment areas, check dietary compliance, and ensure care follows resident care plans and wishes.
  • Meet with residents and family members, respond to complaints and grievances, and report problem areas to the Nurse Supervisor.
  • Inform families of resident deaths, contact funeral homes when requested, and follow post-mortem procedures.
  • Develop, plan, conduct, and schedule in-service training and orientation for nursing personnel.
  • Standardize work methods, train personnel on blood and body-fluid exposure risks, and assist with clinical supervision for nurse aide trainees.
  • Attend required external, facility, safety, infection-control, advance-directive, and continuing-education programs.
  • Monitor personnel compliance with safety regulations, dress codes, hand hygiene, infection-control procedures, fire and disaster drills, and use of personal protective equipment.
  • Maintain clean, safe, and sanitary nursing work areas and resident care areas.
  • Participate in infection-control programs and procedures for communicable diseases, isolation precautions, hazardous conditions, and chemical safety.
  • Ensure adequate supplies of personal protective equipment, medications, medical supplies, and equipment and recommend unit supply needs.
  • Develop and implement procedures for safe equipment operation and ensure only trained personnel use equipment safely and efficiently.
  • Review resident care plans daily, identify needed changes, ensure nursing notes document care-plan implementation, and ensure certified nursing assistants follow care plans.
  • Assist with planning, scheduling, and revising the Minimum Data Set assessment process, including Resident Assessment Protocols and triggers.
  • Report suspected or known fraud involving false billings, cost reports, and kickbacks and provide data to the Quality Assurance and Assessment Committee.

Dr Conway currently serves as CEO of Optum, UnitedHealth Group's health services division with revenues exceeding $200 billion.
