Requirements
  • A Bachelor's degree in social work or human services is required.
  • Must possess any certifications or licensures required by the state of employment to practice in long-term care.
Responsibilities
  • Oversee the operation of the nursing center Social Services Department according to the National Association of Social Workers Code of Ethics and applicable federal, state, and local guidelines, regulations, policies, procedures, and standards of care.
  • Foster a climate, policies, and routines that enable patients and residents to maximize their individuality, independence, and dignity while achieving the highest practical level of physical, mental, and psychosocial well-being and quality of life.
  • Directly supervise two or more Social Services staff, as applicable.
  • Remain accountable for Social Services practice, measuring intervention outcomes, and departmental productivity.
  • Ensure that a psychosocial assessment is completed for each patient or resident, identifying social, emotional, and psychological needs and strengths.
  • Ensure that patient and resident discharge goals are identified at admission and documented accordingly.
  • Assess each patient and resident for discharge.
  • Educate staff about the role of Social Services, the psychosocial needs of patients and residents and their families or significant others, and the problems of aging and disability.
  • Serve as the manager on duty on rotating weekends with other department managers.
