Requirements
  • California active Registered Nurse license
  • Associate of Science Degree in Nursing
  • Three (3) years of oncology nursing experience
Responsibilities
  • Enhance the patient experience by providing a seamless navigation process from abnormal finding/diagnosis through the cancer continuum.
  • Responsible for collaborating with physicians and members of the interdisciplinary teams to triage, coordinate, and consistently manage patient care by educating the patient and serving as a point of contact for patients and families.
  • Works with patients and families to answer questions, provide emotional support, and resolve any barriers, needs or issues related to the cycle of cancer care.
  • Assesses patients’ current and future needs, provides patient access to resources internal and external, facilitates distress screenings and patient needs assessments, and makes appropriate referrals.
  • Establishes and maintains close collaboration with physicians and the cancer care team to ensure patient care and coordination using oncology nursing knowledge base, experience and expertise.
  • Actively participates in multidisciplinary conference, providing patient follow up as needed.
  • Provides patient education and symptom management support.
  • Participates in community health promotion and awareness programs.
  • Performs other duties as assigned.
Desired Qualifications
  • Oncology Certified Nurse (OCN) is preferred.
  • Ability to problem solving, decision-making and critical thinking; excellent organizational, written and verbal communication and interpersonal skills are preferred.
  • Work in a self directed environment and effectively handle multiple priorities is preferred.
  • Knowledge of the American College of Surgeons (ACoS) Commission on Cancer (CoC) standards is preferred.
  • Bi-lingual in English/Spanish is preferred.
  • Prior experience planning, organizing, and completing individual treatment plans is preferred.
  • Prior experience implementing professional and community based education programs is preferred.
  • Proficient in Microsoft Office, Electronic Health Records (EHR), and related database programs is preferred.
  • Knowledge of medical terminology, CPT coding, anatomy, histology, hematology and cancerrelated diagnostic procedures is preferred.
  • Knowledge of the American College of Surgeons (ACoS) Commission on Cancer (CoC)standards is preferred.
