Requirements
  • Graduated from an accredited nursing program within the past 12 months.
  • Obtain a nursing license and registration within the timeline defined by state regulations or within 90 days of hire, whichever occurs first.
  • Hold a current appropriate state Temporary Permit.
  • Meet the practice requirements in the state of employment.
  • Obtain and maintain all required credentials or certifications in active and valid status.
  • Handle demanding situations, multitask, and remain flexible to changing needs.
  • Maintain regular and punctual attendance.
  • Perform frequent prolonged standing and bending, assist with heavy lifting and patient movement, and move equipment with assistance as required.
  • Take the Ishihara Color Blindness test as a condition of employment; failure does not preclude employment, subject to reasonable accommodation.
Responsibilities
  • Direct Patient Care Technicians and delegate tasks to direct patient-care staff, including licensed vocational/practical nurses, Patient Care Technicians, and Dialysis Assistants.
  • Assess daily patient-care needs and develop appropriate patient-care assignments.
  • Monitor patient-care staff techniques and adherence to facility policies and procedures.
  • Participate in staff training, new-staff orientation, and required staff meetings.
  • Educate patients and families about End Stage Renal Disease, renal disease, vascular access, dialysis therapy, diet, fluid and medication compliance, laboratory values, transplant awareness, and treatment adequacy.
  • Deliver safe and effective hemodialysis care to patients with End Stage Renal Disease.
  • Implement treatment prescriptions, including sodium and ultrafiltration modeling when appropriate.
  • Assess patient responses to hemodialysis, adjust treatment plans as indicated, and communicate concerns to the Team Leader or physician.
  • Perform nursing assessments and ongoing evaluation and documentation of patient-care needs.
  • Participate in pre-evaluation, initiation, monitoring, termination, access homeostasis, and post-evaluation of hemodialysis treatments.
  • Troubleshoot access-flow issues and intervene when patient adequacy status changes.
  • Provide, supervise when applicable, and monitor hemodialysis access care.
  • Administer and monitor prescribed interdialytic transfusions and report adverse reactions.
  • Review, transcribe, and enter physician laboratory orders into the Medical Information System.
  • Prepare laboratory requisitions and ensure correct specimen tubes, collection, processing, packaging, and delivery.
  • Follow up on missed or insufficient laboratory specimens and report abnormal or panic results to physicians.
  • Enforce company policies and state and federal regulations.
  • Maintain safe, efficient, and effective shift operations.
  • Act as a resource for staff members and communicate with the Clinical Manager, physicians, and other healthcare-team members to coordinate patient care.
  • Assist patients with prescription refills and ensure physician orders are entered in a timely manner.
  • Maintain the facility drug list and controlled-substance records.
  • Initiate cardiopulmonary resuscitation and emergency measures when required and ensure emergency equipment is available.
  • Coordinate patient transportation when necessary.
  • Maintain a clean, safe, and sanitary dialysis treatment environment.
  • Operate dialysis-related equipment safely and effectively and ensure patient stations and equipment are clean and properly arranged.
  • Address blood spills according to bloodborne-pathogen control policies.
  • Document physician orders, laboratory results, vital signs, treatment parameters, patient status, and treatment data in the Medical Information System.
  • Implement patient admission, transfer, and discharge procedures and verify Patient Care Technician documentation.
  • Review daily hemodialysis treatment sheets and opening and closing procedures.
  • Initiate and document monthly Continuous Quality Improvement activities and complete monthly nursing progress notes.
  • Maintain complete medical records and patient identification.
  • Complete patient care plans, long-term programs, transplant-status reviews, annual nursing histories and assessments, and annual standing-order reviews.
  • Perform other related duties as assigned.
