Requirements
  • Current Missouri RN licensure and NP License
  • Certification as a Geriatric and/or Adult Nurse Practitioner
  • Palliative Care certification required if working in the palliative care program (May be obtained after employment)
  • Successful completion of a Nurse Practitioner program
  • Must be CPR BLS (basic life support) certified with the American Red Cross
  • Prior experience in Geriatrics preferred
  • Ability to read, analyze and interpret general and professional documents; Ability to write routine reports and correspondence; Ability to effectively present information in one on one and small group situations; Must have proficient computer skills; Must possess the ability to make independent decisions when circumstances warrant such action
Responsibilities
  • Performs physical examinations and institutes preventive health measures
  • Orders, interprets, and evaluates diagnostic tests to identify and assess resident’s clinical problems and health care needs
  • Records physical findings, and formulates plan and prognosis, based on patient's condition utilizing electronic health records. Maintains medical records and submits at least 10% of records to the Medical Director for bi-weekly review in accordance with the “Collaborative Practice Agreement”
  • Records all services performed for accurate billing for services provided in compliance with all local, state and federal laws, as well as payor rules and regulations; Submits billing documents to billing agency in a timely fashion
  • Works with the Medical Director, other in-house physicians, staff, residents and family members to develop a comprehensive resident care plan and reviews plan periodically with resident’s physician
  • Prescribes, recommends, or discontinues drugs and treatments in accordance with the “Collaborative Practice Agreement” and the state’s “Nurse Practice Act”; Assists EverTrue in establishing formularies for medication and other therapeutic measures
  • Prescribes therapy such as physical therapy, respiratory therapy, or related therapeutic procedures
  • Refers patients to Physician for consultation or to specialized health resources for treatment with a goal of achieving better outcomes in multi-disciplinary treatment plans; Certifies and re-certifies resident for Medicare stays
  • Looks for ways to improve and promote quality; Provides bedside teaching and/or clinical education programs to clinical staff if requested; Collaborates and advises with EverTrue Home Office clinical team in establishing best practices; Collects and analyzes data to identify best practices, state averages, and Medicare guidelines
  • Maintains credentials and licensure with all major insurance companies and licensing boards with the assistance of EverTrue and billing agency
  • Follows all procedures and protocol as per EverTrue policy, federal and state regulations, standard practice and in accordance with the “Collaborative Practice Agreement”
  • Responds to emergencies in licensed areas, or in Independent Living, when available
