Requirements
  • Licensed Vocational Nurse (LVN) with California license
  • Computer skills sufficient to meet production demands of the job, including Microsoft Office, TIER, MyEvolve software, and knowledge of the Office of Refugee Resettlement UC Portal
  • Ability to maintain detailed, accurate and complete records
  • Effective oral and written communication skills; bi-lingual skills in Spanish/English preferred (note: preferred, not required)
  • Strong organizational and case coordination skills
  • Ability to manage confidential health information
  • Sensitivity in engaging with clients, families, treatment teams, and staff without regard to race, religion, ancestry, perceived or actual sexual orientation, and gender identity or expression
  • Knowledge of emergency procedures of the agency

  • The incumbent must be able to perform the job safely, without endangering the health or safety of themselves or others
  • Minimum two (2) years in residential care, behavioral health, medical coordination, or child welfare settings; experience with ORR, DHCS, DMH, or DHS documentation standards strongly preferred
Responsibilities
  • Provide advanced non-medical/clerical case coordination and health services support for youth placed in ORR-funded residential shelter care.
  • Serve as central liaison between the Health Center, residential staff, clinicians, external medical providers, and the multidisciplinary treatment team to support youth health outcomes within a trauma-informed, culturally responsive framework.
  • Ensure timely access to medical, dental, behavioral health, and specialty services while maintaining compliance with ORR policies, DHCS standards, HIPAA requirements, and agency protocols.
  • Maintain detailed, accurate, and complete records related to health services and case coordination.
  • Communicate effectively with clients, families, treatment teams, and staff, with consideration for language and cultural differences; bi-lingual Spanish/English preferred.
  • Engage with clients, families, treatment teams, and staff with sensitivity toward race, religion, ancestry, perceived or actual sexual orientation, gender identity or expression.
  • Be able to work independently as well as collaboratively within a team; demonstrate organizational skills and proactivity.
  • Follow and implement emergency procedures of the agency.
  • Perform other duties as assigned
Desired Qualifications
  • Bilingual Spanish/English preferred
  • Minimum one year experience in Social Services, including DMH documentation is preferred
  • Experience with ORR, DHCS, DMH, or DHS documentation standards strongly preferred
