About the Role

We’re looking for a Backend Engineer to help build the infrastructure powering intelligent, agentic systems for healthcare. In this role, you'll focus on architecting reliable, scalable services and data pipelines that support AI agents, real-time workflows, and complex operational logic. You won’t need to build models yourself, but you’ll work closely with AI engineers and product designers to integrate LLM capabilities into production systems. This is a hands-on Individual Contributor (IC) role for someone who thrives in fast-paced environments, enjoys building clean APIs, and wants to shape the foundational backend systems of a high-impact, mission-driven product.

Responsibilities

• Design AI interfaces and agent workflows (chat, tools, actions) end-to-end—from problem framing and user flows to high-fidelity Figma and production QA.
• Collaborate with AI engineers to design interfaces that handle real-time data and dynamic outputs.
• Prototype rapidly using Figma (advanced prototyping), React, Replit, and Cursor; when helpful, use lightweight code in HTML, CSS, and JavaScript or Framer; run usability tests, instrument analytics, and iterate based on controlled experiments and session reviews.
• Simplify complex lab tasks into intuitive layouts and user flows.
• Drive innovation by exploring new AI design opportunities and creating 0→1 designs that push the limits of creative AI applications.
• Contribute to the long-term vision of AI at 3Y, shaping the roadmap and scaling AI capabilities across the company.

Qualifications

• 5+ years of backend engineering experience, ideally building production systems for complex or data-heavy applications. 
• Strong experience with modern backend frameworks and languages (Java, Python, TypeScript/Node.js). 
• Experience designing scalable APIs and distributed systems architecture. 
• Solid understanding of database design (PostgreSQL, Redis, etc.), data modeling, and performance optimization.
• Comfortable working in a startup environment with evolving priorities and high ownership expectations. 
• (Preferred) Exposure to LLM workflows, agentic design patterns, or function calling APIs — or an interest in learning. 
• (Bonus) Experience with healthcare data, compliance concerns (e.g., HIPAA), or operational tooling is a plus. Based in NYC or willing to work full-time in our NYC office with a fully in-person team.
• Experience working with cross-functional teams.
• Ability to work 5 days a week in our San Francisco and New York office with a fully in-person team.

Compensation 

The estimated salary range for this role is $250,000 - $300,000. Total compensation for this position may also include stock options. Note that total compensation for this position will be determined by each individual’s relevant qualifications, work experience, skills, and other factors.
