US hospitals lost $48B to denials and bad debt in 2025, up 25% from previous year

Healthcare providers on the Kodiak Solutions platform lost $48 billion in net revenue to final denials and bad debt in 2025, up 25% from $38.6 billion in 2024, according to the company's latest benchmarking analysis of over 2,300 hospitals. The median final denial rate increased from 2.5% to 2.7%, whilst bad debt rose from 1.1% to 1.3%. Clinical denials—covering lack of prior authorisations and medical necessity—accounted for virtually all the increase. Medicare Advantage plans showed denial rates more than double those of traditional Medicare. Patient responsibility increased from 6.8% to 7.3% of net revenue, whilst collection rates declined from 45.1% to 42.4%. However, hospitals saw improvements in cash flow metrics, with reduced payment times and accounts receivable days.
