Requirements
  • One to three months of experience in a human services setting; in some cases, experience may be substituted for academic training.
  • A high school diploma or equivalent is required.
  • A valid United States driver's license must be presented at employment if driving is required.
  • Drivers must be at least 21 years of age, have maintained a valid United States driver's license for at least six months, pass an annual driver-screening background check, and maintain a safe driving record.
  • Employees using a personal vehicle must possess and maintain adequate insurance.
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, sit, reach, climb stairs, and maneuver through narrow spaces or hallways.
  • Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift.
  • Ability to operate a computer, calculator, copier, and printer.
  • Ability to assist clients with tasks of daily living.
  • Ability to speak, hear, and communicate with clients, staff, and external representatives.
  • Ability to meet the specified close, distance, color, peripheral, and depth-vision requirements and adjust focus.
  • First Aid certification is required within two weeks of hire.
  • Cardiopulmonary resuscitation certification is required within two weeks of hire.
  • Medication Administration Program certification is required within 150 days of hire.
  • Safety Care certification is required within 90 days of hire.
  • Knowledge and use of advocacy techniques, communication styles, empowerment and self-advocacy techniques, physical personal management skills, household management skills, and crisis intervention techniques.
  • Ability to document interventions, work independently and as part of an interdisciplinary team, and provide role-model behaviors.
Responsibilities
  • Assist in developing, implementing, and documenting individual treatment support plans.
  • Perform daily tasks with persons served, including habilitative and rehabilitative interventions, behavioral interventions and monitoring, scheduling appointments, monitoring progress, providing transportation, and related tasks.
  • Provide role-model support to persons served and assist them in becoming fully integrated into their communities.
  • Assess the physical and emotional status of persons served and report changes to a supervisor.
  • Ensure program-site safety and maintenance and oversee and participate in daily living and cleaning activities, including cooking, meal planning, grocery shopping, housekeeping, lawn mowing, snow shoveling, and garbage removal.
  • Comply with corporate and divisional policies and procedures, including safe work practices, workers' compensation, universal precautions, and exposure control.
  • Store, administer, document, and dispose of medication for persons served in accordance with Medication Administration Program requirements, funding-source regulations, and company policy.
  • Complete communication logs, community membership logs, chore schedules, timesheets, and application forms.
  • Perform crisis intervention as needed.
  • Provide the Relief Pool Coordinator or Human Resources staff with current telephone, email, and mailing address information.
  • Perform other related duties as required.
