Clinical Pharmacist

Job Details

Under the direction of the Director or Manager the Pharmacist assumes responsibility for the pharmacotherapy outcomes of patients through knowledge and skills coupled with innovative technology and the packaging and distribution of medications.

Responsibilities

• Serves as a primary point of contact for patients, families, pharmacists, healthcare providers, and clients to address same-day medication-related problems (MRPs), pharmacotherapy needs, dispensing issues, and related questions.

• Provides education on care plans, medication use, side effects, and contraindications verbally and/or through written communication within proprietary software.

• Ensures accurate, timely, and compliant medication fulfillment and distribution using proprietary systems, including proper documentation of patient care decisions, billing, and coding.

• Oversee Pharmacy Technicians to ensure appropriate medication reconciliation, physician order entry, inventory management, medication storage and security conditions, and medication labelling and packaging as well as compliance to company policies and procedures and regulatory guidelines.

• Assesses new prescriptions and refills using professional judgment; makes recommendations to care providers to optimize safety and patient outcomes.

• Monitors and supports Community Pharmacy Network (CPN) operations, including ATM dispensing, local medication procurement, CPN transfers, and documentation of transactions. Provides local pharmacies with necessary prescription documentation and billing information.

• Uses professional judgment to provide timely, accurate and appropriate pharmaceutical care appropriately assessing new medication requests and refills making recommendations, in writing or via telephone to care providers to enhance patient outcomes and/or safety as needed in relation to care plans, changes, issues, and education.

• Ensures compliance to state and federal laws and applicable rules and regulations as well as internal policies and procedures required for pharmacy practice.

• Reports and works to rectify or prevent any incidents of QAs, HIPAA violations, Fraud, Waste and Abuse or other problems or violations.

Education:

• BS in Pharmacy, PharmD preferred.

• A current, active license in the state on New Juersey

Experience:

• 2+ years of experience required in pharmacy distribution and packaging, with at least some exposure to geriatrics and/or chronic care management. 

• Experience or strong interest in clinical pharmacogenomics is preferred.

• Strong analytical skills with good clinical judgement and decision making including (1) the ability to effectively and efficiently identify and rectify medication-related problems, and (2) assess patients using verbal and/or visual cues, when appropriate. 

• Knowledge of state and federal laws and regulations, including HIPAA and Fraud, Waste, and Abuse. 

• Proficient with MS Office Suite and strong computer proficiency to navigate proprietary and other software. 

• Excellent communication skills, both verbal and written.

• Ability to communicate effectively and professionally through written, verbal, and interpersonal skills as applied when interacting with employees, clients, or agency representatives; successfully conveying and exchanging information in a positive manner.

Physical & Mental Demands

This position is administrative in nature and will present physical demands requisite to a position requiring: hearing, seeing, sitting, standing, talking, and walking. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Must be able to commute to multiple site locations within assigned territory. May be necessary to work extended hours as needed.

Schedule

• Tuesday-Saturday 3:30PM-12AM ET

• This is a full-time position with an expectation to work an average of 40 hours per week and be available outside of normal business hours to meet customer expectations on an ad-hoc basis. Schedules are set to accommodate the requirements of the position and the needs of the organization and may be adjusted as needed. Rotating weekend schedule required approximately up to four times a year or as needed along with a rotating holiday schedule.
