JOB DESCRIPTION:

Summary of Responsibility

In keeping with the Mission/Vision/Values of Centerstone, a QIDP provides case management for individuals with developmental disabilities , advocates and evaluates treatment to developmentally disabled and mentally ill service recipients. This includes, advocating and evaluating treatment to developmentally disabled and mentally ill service recipients , maintenance of appropriate documentation.  A QIDP is required to use independent (or works under the direction of supervisor) when performing job tasks/providing services.   As part of Centerstone’s team-oriented philosophy, a QIDP is expected to actively participate in variety of cross-functional teams and performance improvement initiatives.

ESSENTIAL DUTIES

Completes job requirements with independent action and resourcefulness and seeks out additional duties during periods of downtime.  Organizes time, effort, and use of resources to achieve desired outcomes.

Treatment must be recovery focused taking into account trauma informed care practices.  Provides consistent, personal growth-focused services to all assigned clients by maximizing time spent working directly with clients to assist their personal growth process.  To demonstrate clients are the first priority, ensure hours of care expectations are met and time is dedicated to providing personal growth services to clients, their families and collateral contacts.

Assist service recipient in developing an individualized treatment plan, must be person centered, family centered, and service centered.  Monitor service recipient progress in treatment and participate in treatment plan reviews as assigned.  Update treatment plans as appropriate. Submit treatment plans at all stages for medical review, if applicable.

Conduct intake for recipient entering services, including explaining services, gathering information needed on intake materials, ensuring service recipient understands all forms, assuring signature on appropriate contracts and consents, and submitting materials for processing, as assigned. Conduct assessment and write assessment summaries, as assigned.

Assist service recipients with making linkage with internal and external resources as outlined in treatment plan and in coordination with Integrated Healthcare Services.  And maintain appropriate contact with referral sources including providing information on service recipient progress, behavior, and needs (with appropriate releases), and maintaining ongoing consultation when appropriate. 

Assist service recipient in making discharge plans and provide follow up services as assigned

Demonstrates an ability to manage crisis situations, including danger to self and others, to ensure the safety and stabilization of clients and to make appropriate referrals/linkages when clinically indicated.  Does not leave crisis situation until client is in a safe environment or crisis is resolved.

Attend staff meetings, clinical staffings, and other internal meetings as assigned.  Participate on committees as assigned, including quality assurance functions.  Maintain appropriate communication with other employees and work as an effective member of treatment team.

Meet and maintain applicable certification (CEU’S) and licensure requirements.  

OTHER DUTIES AND RESPONSIBILITIES

Market services to external resources as appropriate. Participate in training designed to enhance job-related skills. Provide community education as assigned. Perform other duties, not inconsistent with qualifications and regular duties, as assigned.

RESPONSIBILITY

Supervisory Controls:  The Assistant Director of Vocational Services assigns work priorities, provides ongoing clinical supervision, and interprets and enforces policies and procedures. 

Guidelines:  Board, Administrative and program policies, licensure, accreditation, statutory and regulatory standards restrict the position’s discretion in completing job tasks. Staff has the prerogative to propose changes in practice, procedures and policies that affect their team.

DIFFICULTY

Complexity:  Position is characterized by a series of  program development, crisis management and other decision-making tasks are frequently complex in nature, diligent effort is made to make daily training tasks structured and routine

B.        Scope & Effect:  This job affects the physical, emotional, and psychological well being of service recipients. 

PERSONAL RELATIONSHIPS

A.        Personal Contacts:  Position has daily contact with program staff, Case Managers, residential program staff, regular contact with transportation personnel, parents and or guardians, other direct service staff and support staff and may have occasional contact with external monitoring agency’s

Purpose of Contacts:  The purpose of contacts with the Corporation are primarily to improve service recipient’s services. The purpose of contact with others is primarily to gather and exchange information. 

ENVIRONMENTAL DEMANDS

Physical Requirements:  The work is primarily sedentary, but requires skill in non-violent crisis intervention.  Must be able to lift up to 25 lbs.

B.        Work Environment:    Work is primarily performed in an office environment or community setting such as a hospital, police department, county jail, school, service recipient home, etc.  There is risk due to location of services and/or potential violence of service recipients in crisis.  Position may require contact with bodily fluids; this may require the use of protective clothing.

MINIMUM CREDENTIALS/REQUIREMENTS

The QIDP position requires a Bachelor’s Degree in Human Services area (i.e. Psychology, Social Work, etc.), plus one or more years experience working with individuals who are developmentally disabled.  The QIDP must also successfully complete any testing and/or background clearance required to meet agency and/or funding source requirements.  Direct Service Providers utilizing electronic health records and/or other automated/electronic documentation systems are required to have computer and keyboarding experience and skills or the ability to develop such skills within required timeframes in order to meet the level of proficiency set by the agency.   Direct Service Providers utilizing electronic/automated documentation systems will be required to undergo a computer and keyboarding assessment test to determine proficiency
