Requirements
  • A high school diploma or General Educational Development (GED) credential is required.
  • Must meet Center for Medicare and Medicaid Services (CMS)-approved state and/or national certification requirements within the required state or CMS timeline.
  • Must complete all applicable state licensure, education, and training requirements.
  • Must successfully complete the organization’s dialysis training program and CPR certification.
  • Must be able to perform direct patient care involving heavy lifting, moving patients, assisting with ambulation, prolonged standing, bending, and lifting solutions weighing up to 40 pounds.
  • Must pass the Ishihara Color Blindness Test as a condition of employment; reasonable accommodations may be considered.
  • Must travel between assigned facilities and various locations within the community, with possible travel to regional, business unit, and corporate meetings.
Responsibilities
  • Provide safe and effective dialysis therapy under the direct supervision of a licensed nurse and in accordance with organizational policies, procedures, training, and applicable regulations.
  • Set up and operate hemodialysis machines.
  • Assist health care team members with patient-specific education regarding adequacy measures, disease processes, and vascular access.
  • Welcome assigned patients and inquire about their wellbeing since their last treatment.
  • Evaluate vascular access for patency, perform vascular access cannulation, and administer heparin as delegated or permitted by state law.
  • Obtain pre- and post-treatment vital signs and weights and perform vascular access evaluations.
  • Initiate and discontinue dialysis treatments according to prescribed orders and established procedures.
  • Evaluate intradialytic problems, provide prescribed interventions, and monitor patients’ responses to dialysis therapy.
  • Calculate and enter dialysis machine programming for ultrafiltration goals, treatment time, and ultrafiltration modeling as prescribed.
  • Ensure appropriate safety alarms are enabled, including Narrow Venous Limits at 160.
  • Record accurate and timely vital signs, treatment parameters, evaluations, interventions, and patient responses in the Hemodialysis Treatment Sheet and applicable progress notes.
  • Report significant information, changes in patient condition, unusual findings, and vascular-access issues to the registered nurse or supervisor.
  • Observe patients and conduct machine safety checks according to facility policy.
  • Obtain prescribed laboratory testing and prepare, collect, label, store, and arrange courier collection of laboratory specimens.
  • Clean blood spills with appropriate disinfectant and clean and disinfect dialysis machines, chairs, equipment, and surrounding areas between treatments.
  • Prepare, organize, and efficiently use supplies and equipment to prevent wastage.
  • Monitor patients performing self-care under registered nurse supervision.
  • Assist in training applicable direct patient care staff under nurse supervision.
  • Maintain a safe, clean, and aesthetically appropriate environment.
  • Perform all preparation tasks and required pretreatment dialysis machine alarm testing, including Pressure Holding Tests.
  • Initiate the Solution Delivery System.
  • Complete and document participation in Continuous Quality Improvement activities.
  • Enter treatment data into the designated clinical application.
  • Review treatment sheets for completeness and ensure nursing signatures and omitted entries are completed or corrected by appropriate staff.
  • Prepare initial patient charts for admission and compile paperwork for chart placement.
  • Collect information for infection control audits and support staff and patient adherence to infection control practices.
  • Follow infection control policies and procedures and reinforce infection control education for patients and families under the direction of the Clinical Manager or designee.
  • Follow newly admitted patients through their first four weeks of dialysis and coordinate transitional dialysis care.
  • Reinforce education and care information for new patients as permitted by state law.
  • Liaise with appropriate facility staff to coordinate the patient experience and ensure disciplines are scheduled according to plan.
  • Assist patients in understanding home dialysis products, benefits, and treatment options, including demonstrating peritoneal dialysis setup.
  • Set up and perform treatments on all machines used in the Transitional Care Unit.
  • Assist with coordinating transitions to the patient’s chosen dialysis modality.
  • Assist with supply ordering and inventory for the Transitional Care Unit.
