Requirements
  • Valid State LPN License
  • Valid driver’s license and use of automobile is required due to the travel involved
  • Annual MUI (Incident) training as dictated by the State of Ohio Medicaid, Ohio Home Care Program and Department of Developmental Disabilities
  • Must be able to read and understand various documents used by the agency including Plan of Care, flow charts, client charts, agency policies and procedures
  • Must be able to use a computer, laptop or tablet
  • Must be able to think independently, recognize potential problems, gather information, and know when to contact a supervisor
  • Employees must promote excellent customer service and always provide exceptional client care, while staying within the rules of the agency and rules of each program served
Responsibilities
  • The LPN will administer medication in accordance with standards of practice.
  • The LPN will encourage social and rehabilitative skills and encourage self-care and independence.
  • The LPN will perform client treatments in accordance with the Plan of Care.
  • The LPN will assist the consumer in learning appropriate self-care techniques.
  • The LPN will prepare clinical and progress notes to document care provided.
  • The LPN will monitor, document accordingly and report any change in client condition to Clinical Director/Clinical Supervisor.
  • The LPN will promote activities to achieve a safe and healthy environment.
  • The LPN will observe aseptic technique when performing client procedures/care.
  • The LPN will collaborate with other members of the clinical team to develop and improve the Plan of Care.
  • The LPN will assist the client in achieving established goals in the Plan of Care developed by the RN.
  • The LPN will maintain productivity according to the standards of the agency.
  • The LPN will maintain license, CEUs and mandatory employment requirements.
  • The LPN will participate in client care meetings, in-services, orientation and staff meetings.
  • The LPN will provide service in accordance with the agency’s philosophy, standards, policies and scope of practice.
  • The LPN will participate in Performance Improvement and adheres to HIPAA guidelines.
  • Other duties as assigned.
