Community Support Coordinator

Description

Purpose:

The Community Supports Coordinator (CSC) will be responsible for assisting people with approved HCBS Waiver Community support; this can include, but is not limited to Respite (group, site based and in-home supports) and Community Habilitation. The CSC will participate in all Life Plan meetings and educate the team about the current regulations and specific policy and procedures. This is an opportunity for the team to familiarize themselves with AIM specific policies and for the CSC to assist with any questions. The CSC will ensure that all community supports are person centered and reflect the person’s goals and needs.

Requirements

Qualifications:

· Associate’s degree in a Human Services Related Field with 2 years’ experience working with individuals with disabilities preferred; OR High School Degree with 4 years’ experience working with individuals with disabilities preferred.

· Program specific training (provided)

· Valid NYS Driver’s License acceptable to Agency Insurance standards. Maintain valid insurance and provide insurance card if using your own vehicle.

· Ability to exhibit/maintain a professional, kind, courteous and patient manner at all times.

Physical/Cognitive Requirements:

· Lifting – individuals/items of various weights (minimum 50 pounds).

· Excellent verbal and written communication skills.

· Use of computer for extended periods of time.

· Ability to remain calm in stressful or demanding situations.

· Demonstrated interest and ability to make possible the personal outcomes of individuals receiving services

· Adherence to Agency policies and procedures

· Traveling to multiple sites in the provision/coordination of services.

Responsible To: Assistant Director of Community Supports /Designee

Status: Non- exempt

Essential Functions:

· The CSC will explain all required waiver documentation to the person being supported. They will provide the team with copies of all written policies and procedures.

· The CSC will compose and maintain current documents for each participant including, but not limited to, a current signed Life Plan identifying AIM Services Inc as the community supports provider of service, a current Staff action plan, any necessary plan attachments, and a Community Habilitation Monthly Summary Note.

· The CSC will maintain billing documents for each participant they support and submit according to standards set by the agency.

· The CSC will assist with supervision of staff and training as needed. The CSC will provide direct support as requested/needed.

· Work with the entire Community Supports Department to assure all services remain person-centered and the participants are accessing the supports offered through AIM Services.

· Participate in all Life Plan meetings for all participants they are supporting

· The CSC will be the contact point for all questions and concerns for anyone supported through the AIM Services Community Supports program.

· Assist with oversight and managing assigned Respite/ Community Habilitation programs from development to implementation.

· Assist with the routine observation of support staff, provide constructive feedback in a timely manner and participate in the completion of their observations and evaluations.

· Develop person centered goals and objectives tailored to enhance each person’s strengths and provide training, support and resources necessary to achieve those goals.

· Assist with routine contact with people supported as well as family to discuss and evaluate service plans and to address any concerns or issues.

Other Duties:

· Flexibility of schedule to ensure support and program implementation.

· Any and all other related duties as requested by supervisor.
