Requirements
  • Graduate of an accredited School of Nursing.
  • Hold current appropriate state licensure and meet practice requirements in the state of employment.
  • Hold current or successfully complete CPR Basic Life Support certification.
  • Have a minimum of 2 years of nephrology nursing experience within the last 2 years as a registered nurse.
  • Complete required Clinical Advancement Program requirements to maintain or advance.
  • Be able to provide direct patient care involving heavy lifting, moving patients, assisting with ambulation, prolonged standing, bending, and occasional movement of equipment up to 200 pounds and chemical or water solutions up to 30 pounds.
  • Participate in on-call, night, weekend, holiday, or program-defined coverage rotations.
  • Be able to travel to training sites, other facilities, and patient homes when required.
Responsibilities
  • Provide and coordinate clinically competent care, including assessment, planning, intervention, and evaluation, for an assigned group of patients.
  • Collect and analyze dialysis data for assigned patients, document it in medical records, modify treatment plans as indicated, and notify the supervisor or physician as needed.
  • Assess, collaborate on, and document patient and family learning needs and provide initial and ongoing education.
  • Direct and provide safe and effective dialysis therapies in collaboration with patients, home care partners, and patient care staff.
  • Administer prescribed medications or medications authorized by approved algorithms and document medical justification and effectiveness.
  • Initiate or assist with emergency response measures.
  • Serve as a resource, leader, coach, mentor, and role model for new and incumbent employees.
  • Ensure correct laboratory collection, processing, and shipping procedures and reschedule missed or insufficient collections.
  • Identify expected outcomes and document and update nursing assessments and plans of care in collaboration with the interdisciplinary team.
  • Educate patients about transplant and treatment modality options.
  • Identify, evaluate, select, and educate home dialysis candidates and home partners.
  • Assess barriers in the candidate's home environment and evaluate partner or family readiness and ability to perform home dialysis.
  • Train home dialysis patients and home partners to safely operate and maintain home dialysis equipment and treatment supplies through the standardized training program.
  • Participate in facility- and area-level education and quality improvement projects.
  • Serve as a preceptor to new employees when assigned.
  • Provide assigned oversight of registered nurses, licensed practical or vocational nurses, patient care technicians, and home therapies care team assistants as a team leader or designated nurse in charge after completing required training and obtaining the required experience.
  • Rotate coverage with other licensed home therapy staff to ensure reliable and adequate coverage.
  • Perform other duties assigned by the supervisor.
