Requirements
  • Registered Nurse licensure and ability to perform duties within the scope of practice outlined by the state.
  • Ability to work autonomously and manage time efficiently during after-hours coverage.
  • Ability to perform the listed job functions without limiting physical or medical conditions.
  • Ability to follow agency policies, procedures, rules, regulations, Medicare, and federal guidelines.
  • Availability for weekday evening hours and overnight on-call coverage, with schedule changes communicated to schedulers or supervisors.
Responsibilities
  • Respond to after-hours patient needs and provide skilled nursing care in patients' homes as directed by the attending physician.
  • Provide one-on-one patient care and support patients, families, nurses, and the interdisciplinary team.
  • Observe and monitor patient conditions and perform OASIS assessments to develop and adjust individualized care plans.
  • Administer medication as prescribed by the physician and prioritize visits for high-risk patients to help decrease rehospitalizations.
  • Oversee and supervise the total care of patients provided by nurse aides and licensed practical nurses.
  • Manage multidisciplinary care and promote continuity through appropriate admissions, transfers, and discharges.
  • Educate patients and families about disease processes using teach-back methods.
  • Complete clinical documentation according to agency protocol and Medicare and federal guidelines.
  • Collaborate with the interdisciplinary team and physicians to ensure optimal patient care.
  • Attend in-service training and mandatory agency meetings.
  • Complete chart reviews and patient phone calls when after-hours needs are minimal or absent, supporting patient care and regulatory compliance.
  • Communicate changes in schedule and availability to schedulers or supervisors.
