Requirements
  • Active and unencumbered South Carolina Licensed Practical Nurse license
  • Current CPR certification
  • Graduate of an accredited Practical Nursing program
  • Long-term care experience preferred, but not required
  • Strong clinical and communication skills

Responsibilities
  • Provide direct nursing care to residents
  • Supervise the day-to-day nursing activities performed by nursing assistants
  • Administer medications and treatments as ordered by physicians
  • Monitor resident conditions and report changes appropriately
  • Collaborate with CNAs, nurses, physicians, and interdisciplinary team members
  • Maintain accurate and timely documentation
  • Help create a safe, comfortable, and caring environment for residents
  • Build meaningful relationships with residents and their families
  • Direct the day-to-day functions of the nursing assistants in accordance with current rules, regulations, and guidelines that govern the long-term care facility
  • Ensure that all nursing personnel assigned to you comply with the written policies and procedures established by this facility
  • Periodically review the department’s policies, procedure manuals, job descriptions, etc., and make recommendations for revisions
  • Meet with assigned nursing staff and support personnel to plan shifts’ services, programs, and activities
  • Ensure Nursing Service Procedures Manual is current and reflects daily nursing procedures
  • Ensure nursing service personnel comply with procedures in the Nursing Service Procedures Manual
  • Make written and oral reports/recommendations concerning shift activities
  • Cooperate with other resident services when coordinating nursing services to maintain total care regimen
  • Admit, transfer, and discharge residents as required
  • Complete accident/incident reports as necessary
  • Write resident charge slips and forward to the Business Office
  • Maintain Daily Census Report and submit to Business Office as required
  • Perform administrative duties such as completing medical forms, reports, evaluations, etc.
  • Directs nursing staff in compliance with job descriptions
  • Participate in development and maintenance of quality assurance program for the nursing service department
  • Participate in facility surveys by government agencies as requested
  • Assist in planning nursing services portion of resident discharge plans
  • Interpret department policies to personnel, residents, visitors, and government agencies
  • Complete and document discharge planning activities; plan discharge services as necessary
  • Charting and Documentation related duties as listed (receiving orders, transcribing, charting nurses notes, filing forms, submitting accident reports, etc.)
  • Prepare and administer medications; verify resident identity; ensure narcotic records accuracy; dispose of drugs per procedures
  • Order medications, supplies, and equipment as necessary; ensure adequate floor stock
  • Notify physicians of stop orders and discrepancies; ensure safe administration of medications
  • Provide leadership to nursing personnel; perform daily rounds; ensure CNAs are trained and supervised
  • Verify CNA training program enrollment and supervision
  • Attend meetings with nursing staff to improve services
  • Comply with facility policies; manage disciplinary actions fairly
  • Inform staff and residents of admissions and room assignments; participate in orientation of new residents and families
  • Assist in coordinating diagnostic and therapeutic services; consult with physicians as necessary
  • Review care plans daily; inform Nurse Supervisor of changes; ensure CNAs refer to care plans before care
  • Assist in planning and revising MDS, RAPs, and Triggers
  • Participate in safety and sanitation programs; maintain infection control programs; maintain PPE
  • Ensure adequate stock of medications, supplies, and equipment
  • Develop and maintain care plans, goals, problems, approaches, revisions; ensure CNAs refer to care plans
  • Assist in budgeting and planning related to nursing services
Desired Qualifications
  • Knowledge and experience with PCC (Prairie Community Care PCC stands for Palette PCC In healthcare PCC often Patient Care Coordination or Personal Care Coordinator; assume PCC electronic charting).
  • Long-term care experience preferred (already in requirements)
  • Leadership and supervisory ability as a desirable trait

PACS Group has acquired operations of three post-acute care facilities totalling 230 beds—two in Alaska's Kenai Peninsula region and one in Boise, Idaho.
