Requirements
  • A bachelor's degree in a healthcare-related field, social service field, or RN.
  • Two years of healthcare or hospital experience as a Registered Nurse or Social Worker is preferred.
  • Knowledge of basic computer skills and word processing.
  • A license as a Registered Nurse or Social Worker in the state of employment.
  • Must obtain Basic Life Support certification within 30 days of employment and maintain it for the duration of employment.
  • Must occasionally lift or move up to 20 pounds and meet the stated visual, standing, walking, manual dexterity, reaching, speaking, and hearing requirements.
Responsibilities
  • Provide care for adult or geriatric patients aged 18 and over.
  • Respect and protect the rights, dignity, and individuality of each patient.
  • Coordinate clinical care for patients, families, and significant others to help them address the impact of illness and maximize health-care benefits.
  • Complete a psychosocial assessment for each new admission within three business days according to hospital policy and procedures.
  • Develop and implement programs addressing the social and psychological factors of illness for patients and families.
  • Plan patient care services and patient care plans with physicians, nursing personnel, and other members of the management team.
  • Conduct individual or group intervention sessions related to care and adjustment.
  • Arrange discharge and post-hospital care through community institutions and agencies.
  • Provide advice and guidance for special cases or patient needs.
  • Provide patients, families, and hospital departments with information and guidance about post-discharge treatment and recovery programs.
  • Refer patients and families to appropriate community, social-service, home-health, extended-care, and rehabilitation resources.
  • Coordinate case-management communications and serve as the primary liaison with external social-service organizations.
  • Participate in interdepartmental program planning and implementation and support related projects.
  • Participate in case-management education for physicians and other departments regarding medical-necessity aspects of health-care delivery.
  • Represent the department on hospital committees.
  • Develop and implement the Pastoral Care Program with Pastoral Care from the host hospital.
  • Maintain working relationships with other departments and address interdepartmental social-service issues.
  • Assist with preparation of and monitor the department budget, identify variances, and respond to them.
  • Monitor utilization reviews and patient length of stay.
  • Direct departmental quality-improvement activities and promote high-quality, safe, and effective patient care.
  • Prepare statistical reports on department services and patient needs.
  • Follow the Hospital Compliance Plan and report suspected violations to the Compliance Officer.
  • Follow professional-conduct, hospital, departmental, attendance, safety, hazardous-material, and patient-safety policies.
  • Recognize potential patient abuse and follow policy for appropriate referrals and interventions.
  • Participate in Performance Improvement activities.
  • Limit access to protected health information to the minimum necessary and share it only for work purposes.
  • Perform other related duties as assigned.
