Requirements
  • One to two years of experience or training in a related function.
  • A high school diploma or equivalent is required.
  • A valid driver's license, adequate insurance when using a personal vehicle, a safe driving record, at least 21 years of age, at least six months of valid United States driver's license history, and successful completion of a driver's screening background check are required.
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, lift at least 25 pounds using proper lifting techniques or a two-person lift, operate a computer and office equipment, assist clients with activities of daily living, and maneuver through the facility.
  • Ability to speak, hear, and communicate with clients, staff, and external representatives.
  • Close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus are required.
  • Cardiopulmonary resuscitation certification is required within two weeks of hire.
  • First Aid certification is required within two weeks of hire.
  • Medication certification is required within 150 days of hire.
  • Safety Care certification is required within 90 days of hire.
Responsibilities
  • Support management in ensuring a high-quality service environment in the Day Program.
  • Provide individual and group support to persons served in the Day Program.
  • Teach skills that enhance independence and self-advocacy and support choices related to meaningful living.
  • Maintain a safe, secure, and clean Day Program environment.
  • Support individuals in community integration, social-skill development, and preparation for employment.
  • Assist individuals in accessing community-based support and resources.
  • Plan recreational and social activities that promote social inclusion and emotional well-being.
  • Support individuals in making choices, advocating for themselves, and participating in decisions about their supports and lives.
  • Support individuals in securing, maintaining, and developing work, volunteer, and community-engagement opportunities.
  • Schedule staff assignments for community and employment activities.
  • Assist in developing, implementing, and documenting treatment, including Individual Program/Positive Behavior Support Plans and clinical programs.
  • Provide transportation as necessary and schedule morning and evening transportation runs.
  • Procure, store, administer, document, and dispose of medication in accordance with Medication Administration Certification requirements, funding-source regulations, and company policy.
  • Communicate with the program nurse, residential managers, Quality Department members, guardians, and Department of Developmental Services case managers.
  • Participate in mandated training and staff meetings.
  • Conduct follow-up training with newly hired direct-care staff.
  • Ensure compliance with internal and external regulatory policies, procedures, and standards.
  • Ensure facility safety and maintenance, including housekeeping, snow shoveling, and trash removal.
  • Maintain required staff-to-individual ratios and secure coverage when necessary.
  • Set up site-specific orientation for new relief staff and keep the Day Program relief-staff list current.
  • Ensure staff read and sign required Positive Behavior Support Plans and service documents.
  • Function as a human rights advocate.
  • Provide physical intervention or First Aid during behavioral or medical emergencies.
  • Record daily arrival and departure attendance in Office 360.
  • Ensure staff and individuals remain in assigned cohort areas and that daily schedule changes are accurately recorded.
  • Schedule vehicle maintenance and repairs, supervise accident reporting, and ensure completion of required paperwork.
  • Report monthly mileage and manage gas cards for each vehicle.
  • Assist staff with incident reports, antecedent-behavior-consequence records, and required paperwork.
  • Support crisis situations.
  • Perform other related duties as required.
