Requirements
  • A current unencumbered Ohio license to practice as a Licensed Practical Nurse (LPN) is required.
  • Candidates must be able to read, write, speak, and understand English.
  • Candidates must be able to lift, transfer, push, or pull residents, patients, or equipment and supplies weighing up to 50 pounds.
  • Candidates must be available to work weekends, evenings, and holidays as needed and be on call 24/7 for emergencies.
  • The role involves exposure to falls, equipment burns, odors, cuts, infectious waste, disease conditions, AIDS, and Hepatitis B viruses, with bloodborne disease exposure risk category I.
Responsibilities
  • Direct the day-to-day nursing activities and make work assignments for Nursing Assistants in accordance with applicable rules, regulations, guidelines, and company standards.
  • Coordinate nursing services to ensure continuity of resident care.
  • Administer medications and treatments according to physician orders and nursing standards of practice.
  • Represent the nursing department in Interdisciplinary Care Plan meetings and answer questions about care plans and service delivery.
  • Participate in committee, employee, departmental, and in-service training meetings.
  • Meet regularly with Nursing Assistants to identify and correct problem areas and improve care and services.
  • Provide direct nursing care as needed.
  • Consult with residents’ physicians about resident care, treatment, and rehabilitation.
  • Notify residents’ physicians and families when a resident’s condition changes.
  • Authorize and monitor restraints when necessary according to facility policies and procedures.
  • Admit, transfer, and discharge patients as necessary.
  • Work with the Unit Manager to help reduce hospital readmissions.
  • Ensure nursing procedures are followed and reflect the facility’s day-to-day nursing care procedures.
  • Participate in maintaining and implementing the facility’s quality assurance program.
  • Ensure nursing staff follow procedures for administering, storing, and controlling medications and supplies.
  • Ensure resident rooms, treatment areas, work areas, and equipment are clean, sanitary, and safe.
  • Supervise State Tested Nursing Assistants (STNAs).
  • Interview, select, and orient new direct reports.
  • Establish and promote performance expectations for direct reports.
  • Evaluate direct reports’ job performance and provide developmental guidance.
  • Monitor employee accountability and work with Human Resources on documentation and intervention when needed.
  • Develop, plan, schedule, conduct, and monitor in-service, on-the-job training, and orientation programs for departmental staff.
  • Maintain a staffing plan for absences or vacant direct-report positions.
  • Review and respond to departmental staff questions, requests, and work products.
  • Work occasional travel and up to six hours of driving per day as required.
Desired Qualifications
  • One year of clinical experience in a hospital, long-term care facility, or related healthcare facility is preferred.
  • Supervisory experience in a long-term care, hospital, or related healthcare setting is preferred.
  • Proficiency with electronic medical records (EMR) is preferred.
