Requirements
  • Board Certified or Eligible in Internal Medicine
  • Board Certified or Eligible in Family Medicine
  • Geriatrics fellowship preferred, but not required
Responsibilities
  • Provide high-quality, comprehensive care in Skilled Nursing Facilities (SNFs) and virtually within the Medicare Home Health Physician program
  • Engage in transitions of care from hospital to SNF and from SNF to home or other settings of care
  • Work as part of an integrated interdisciplinary team with nurses, therapists, social workers, and Kaiser Care Coordinators
  • Care for a spectrum of medical complexities from straightforward to highly complex cases at SNFs
  • Support virtual care model including early, day, and evening shifts and consults via telehealth
  • Support home health field staff in members’ homes and participate in physician consults to determine patient eligibility
  • Assist and participate in the Transition Nurse Team for escalations of patients transitioning from hospitalist discharges or SNF discharges
  • Be part of a call schedule with calls divided equally among providers
  • Work with an advanced Epic EMR and other digital technologies as part of care delivery
  • Opportunity for leadership positions within the medical group if desired
Desired Qualifications
  • Geriatrics fellowship preferred, but not required
  • Experience with digital technologies (AI platforms, telehealth and/or digital health) and adaptability to incorporate into practice
  • Ability to work in both Vancouver, WA and the Portland metro area
  • Public Service Loan Forgiveness (PSLF) eligibility
  • 1.00 and .9 FTE schedules available
  • Relocation allowance up to $15,000
