Requirements
  • Cleansing and support: CHT is required for this role.
Responsibilities
  • Assist in the setup and operation of hemodialysis machines.
  • Assist other health care members in providing patient specific detailed education regarding adequacy measures such as Online Clearance Monitoring, Adequacy Monitoring Program, Urea Kinetic Modeling, and disease process/access.
  • Welcome assigned patients and inquire about well-being; report complaints to nurse supervisor.
  • Evaluate vascular access for patency and report unusual findings.
  • Obtain pre and post treatment vital signs and weight; report unusual findings.
  • Perform vascular access evaluation pre-treatment; report unusual findings.
  • Perform vascular access cannulation; report difficulties or unusual findings.
  • Administer Heparin as delegated or allowed by state law.
  • Initiate dialysis treatment according to orders (QB, QD).
  • Evaluate intradialytic problems and provide intervention as prescribed.
  • Monitor patients’ response to dialysis therapy; report unusual findings.
  • Obtain vital signs prior to reinfusion; discontinue treatment per procedures; document.
  • Discontinue dialysis treatment; evaluate patient prior to termination of venous access (blood pressures).
  • Obtain hemostasis and apply dressings.
  • Evaluate patient prior to discharge and report unusual findings.
  • Perform and record Pre and Post dialysis evaluation, weight and vital signs with initial identification.
  • Calculate and enter dialysis machine programming for Ultrafiltration goal, treatment time, Sodium modeling, UF modeling as prescribed.
  • Ensure safety alarms are enabled including Narrow Venous Limits 160.
  • Record accurate and timely information on Hemodialysis Treatment Sheet with initial identification.
  • Document Hemodialysis Treatment Sheet and progress notes as applicable.
  • Report significant information and changes in patient condition to nurse supervisor.
  • Observe patient and perform machine safety checks; report changes to nurse supervisor.
  • Document interventions for unusual patient status and patient responses.
  • Evaluate vascular access during treatment; monitor pressures; intervene; document and report.
  • Obtain prescribed laboratory testing and prepare specimens for collection.
  • Maintain environmental integrity and aesthetics; keep areas safe and clean.
  • Clean and disinfect dialysis machine surface, chair, equipment, and area between treatments.
  • Prepare for dialysis treatment and document.
  • Perform pretreatment dialysis machine alarm testing including Pressure Holding.
  • Initiate Solution Delivery System.
  • Clean blood spills with disinfectant per policy.
  • Prepare, organize, and utilize supplies to prevent wastage.
  • Complete and document ongoing participation in Continuous Quality Improvement activities.
  • Enter all treatment data into designated clinical application accurately and timely.
  • Review treatment sheets for completeness; ensure nursing signatures; correct omissions.
  • Prepare initial patient chart for admission and assemble paperwork for chart placement.
  • Prepare lab requisitions for prescribed or stat Lab specimens; collect, label, prepare and store specimens as required.
  • Ensure collection of lab specimens by lab courier.
  • Perform additional duties as assigned.
  • For Home Programs: May assist Home Therapies Registered Nurse on home visits; reinforce education; schedule appointments; weigh patients; collect treatment records; prep charts; assist with lab draws; phlebotomy per state regulations; home dialysis setup; clean and prep home treatment areas; etc.
Desired Qualifications
  • Previous patient care experience preferred.
  • CPR certification may be required/needed; note included in requirements as dependent on dialysis training.
