- An active Texas Physical Therapy Assistant license in good standing is required.
- Strong clinical judgment and the ability to work independently in the field within Physical Therapy Assistant scope and under physical therapist supervision are required.
- The ability to escalate changes in condition to physical therapists, registered nurses, and physicians and clearly document the rationale is required.
- Strong communication and documentation skills are required.
- Reliable follow-through on treatment plans, visit cadence, and patient goals is required.
- A current driver's license and comfort with daily travel across the assigned territory are required.
- The ability to carry and transport basic therapy equipment as needed is required.
- Competency in core home health Physical Therapy Assistant scope is required, including transfers, gait training, balance and fall prevention, assistive-device education, strengthening, functional mobility, home-safety assessment and modification education, and patient/caregiver coaching with practical home programs.
- Deliver skilled Physical Therapy Assistant visits in patients' homes under the supervising physical therapist's plan of care.
- Execute treatment plans focused on functional mobility and measurable progress, including gait and balance, transfers, strengthening, functional mobility, home safety, and fall prevention.
- Set and track objective progress within Physical Therapy Assistant scope and communicate updates and concerns to the supervising physical therapist.
- Coordinate multidisciplinary care by escalating changes in condition, aligning on orders, and keeping the care team moving.
- Coach patients and caregivers on practical home programs.
- Document visits promptly and thoroughly in Homecare Homebase.
- Prior home health experience is preferred.
- Experience with Homecare Homebase or a similar home health electronic medical record system is preferred.
- Deeper clinical experience with orthopedic or neurological populations is preferred.
- Strong organization and time-management skills are preferred.
