Mandolin provides an AI-driven automation platform for the specialty pharmaceutical market. Its system uses AI agents that act as a digital workforce and plug into existing EHRs, payer portals, and manufacturer hubs to handle back-office tasks for infused and injected drugs. These agents automate steps like benefits verification, insurance navigation, prior authorizations, parsing documents, and billing to manage the patient access workflow from referral to reimbursement. The platform targets infusion centers, specialty pharmacies, and health systems, aiming to shorten delays to treatment, reduce operating costs, and improve revenue cycle management.

What believers are saying

  • Infusion market growing rapidly with severe staffing shortages driving urgent automation demand.
  • Competitors like SamaCare raising $17M Series B signals strong investor appetite for specialty workflow platforms.
  • Customers report days of work reduced to hours, enabling clinics to scale without hiring.

What critics are saying

  • Epic Systems' native prior auth module in MyChart locks Mandolin out of 80% of clinic EHRs.
  • CMS machine-readable prior authorization rules effective January 2027 bypass third-party platforms entirely.
  • CoverMyMeds acquired InfusionHub AI, consolidating prior auth tools and capturing Mandolin's specialty pharmacy clients.

What makes Mandolin unique

  • End-to-end automation from referral to reimbursement, not point solutions like competitors.
  • Deployed across 700+ clinics serving 250,000+ patients annually with proven 24x speed improvements.
  • Multimodal AI reads unstructured data without requiring integrations or APIs across fragmented systems.
