Requirements
  • A Master of Social Work degree from a school of social work accredited by the Council on Social Work Education is required.
  • Must be minimally licensed in the state where the Oak Street Health Center is located.
  • Fluency in Spanish, Polish, Russian, or another language spoken by people in the communities served is required where necessary.
  • Proficient computer skills, including Microsoft tools, GSuite, and electronic medical records, are required.
  • Must have US work authorization.
Responsibilities
  • Proactively support highest-need patients through comprehensive assessments, telephonic check-ins, and in-person visits.
  • Partner with patients and their support persons to identify goals, increase healthcare engagement, address barriers, and execute care plans.
  • Maintain knowledge of community resources and make appropriate referrals to meet patients' healthcare and psychosocial needs.
  • Navigate patient insurance benefits with the Patient Relations Manager to support mindful utilization and maximize plan benefits.
  • Provide longitudinal and episodic comprehensive care coordination to help patients independently manage health conditions and improve quality of life.
  • Educate patients about supportive care options, including advance care planning and end-of-life care, and provide goals-of-care counseling, support, and advocacy throughout the patient journey.
  • Provide psychoeducation to patients and families about life-stage and disease-specific issues, including dementia, adjustment to illness, aging, and caregiver support.
  • Collaborate with care teams to generate solutions to patient issues and lead interventions involving complex psychosocial dynamics.
  • Complete concise and comprehensive documentation on time so care team members can see the status of Medical Social Worker interventions.
  • Partner with Community Health Workers and provide clinical guidance and consultation regarding complex patient needs.
  • Respond to crises and urgent needs by prioritizing work, triaging situations, and multitasking.
Desired Qualifications
  • Prior experience with complex care coordination and/or population healthcare management.
  • Familiarity with using metrics and data to guide practice and drive outcomes.
  • At least one year of medical social work experience in a healthcare environment, preferably within the last two years.
  • Experience working on a multidisciplinary team of healthcare providers.
  • Experience working with Medicare-eligible populations, including older adults, adults with disabilities, and adults with severe and persistent mental illness.

Nine major PBMs to display TrumpRx prescription drug prices from 2027

Nine pharmacy benefit managers will integrate TrumpRx prescription drug pricing into their benefit tools, effective 1 January 2027. The Pharmaceutical Care Management Association and participating PBMs—including CVS Health, Express Scripts, Humana, and OptumRx—will display cash prices from TrumpRx alongside plan coverage costs. The agreement covers commercial, Medicare, and Medicaid plans. Patients will see TrumpRx prices for all listed drugs, whether through presidential deals or standard pricing. CMS Administrator Dr Mehmet Oz said the commitment will help patients compare prices and find better deals. PCMA CEO David Marin stated the transparency will allow consumers to make better-informed choices about prescription drug costs. Some PBMs will use Real Time Benefit Tools to display pricing comparisons, whilst others may employ different methods.
