Requirements
  • Must possess a valid Licensed Practical Nurse license issued by the state nursing board.
  • Must possess at minimum a high school diploma or General Educational Development credential.
  • Must read, write, speak, and understand English.
  • Must provide annual verification of a negative tuberculosis skin test.
  • Proof of full COVID-19 vaccination is required, with exceptions for medical and religious accommodations.
  • Must meet the facility’s general health requirements, including a medical and physical examination.
  • Must be able to move intermittently throughout the workday and cope with the mental and emotional stress of the position.
  • Must be able to see and hear, or use prosthetics that enable these senses to function adequately.
  • Must be able to work with ill, disabled, elderly, emotionally upset, and occasionally agitated residents.
  • Must be able to push, pull, move, or lift at least 25 pounds to a minimum height of 5 feet and carry that weight at least 50 feet.
  • May need to assist with resident evacuation during emergencies.
  • Must comply with applicable federal, state, and local health and regulatory standards and guidelines, facility policies and procedures, attendance policies, and employment terms.
  • Must maintain compassion, dignity, and adherence to residents’ rights and care standards.
  • Must be available for callback during exigent circumstances such as severe weather, evacuations, or temporary staffing shortages.
Responsibilities
  • Provide care and treatment to assigned residents according to plans of care, established medical practices, applicable laws and regulations, and facility policies and procedures.
  • Administer or assist with medications while adhering to the Five Rights of Medication Administration.
  • Continuously monitor and assess residents’ conditions and report findings appropriately.
  • Reorder and receive medications.
  • Provide personal care and activities of daily living assistance.
  • Supervise care staff and assist with oversight of ancillary care staff when the Wellness Director or Assistant Wellness Director is absent.
  • Maintain shift-to-shift reports.
  • Provide emergency assistance to residents with appropriate follow-up.
  • Obtain and transcribe physician orders.
  • Maintain case management documentation.
