Requirements
- A minimum of a high school diploma or GED and one year of work experience, preferably in a care-providing role, exposure to individuals with developmental or intellectual disabilities, or a combination of experience and education.
- Basic computer proficiency and basic reading and writing skills.
- Communication, interpersonal, charting, and record-keeping skills.
- Ability to accept and provide direction and instruction professionally.
- Ability to pass a background check that includes BCI and FBI criminal history checks.
- Current Med Pass and CPR/First Aid certifications, with onsite training available for CPR and Med Pass.
- A valid Ohio driver's license with fewer than six points and proof of state-minimum vehicle and general liability insurance coverage.
- Ability to stand, walk, sit, use hands, reach, climb stairs, balance, stoop, kneel, crouch, crawl, talk, and hear, and occasionally lift or move up to 50 pounds or partner with a coworker to transfer heavier loads.
Responsibilities
- Provide assistance with individuals' everyday life skills.
- Provide active treatment and emotional and health-related services, including administering medications, assisting with breathing treatments, checking vital signs, monitoring overall health, and assisting with medical equipment.
- Assist with basic personal care and grooming, including bathing, basic hair care, and dressing.
- Complete administrative tasks associated with providing support.
- Administer medication accurately and follow and maintain medication administration records.
- Report incidents or accidents involving individuals immediately to the Site Leader.
- Document all services provided to individuals accurately, legibly, and cohesively.
- Maintain a clean and safe facility and ensure the safety and well-being of individuals.
- Involve individuals in community and recreation-based activities.
- Assist with or complete household chores such as cleaning, laundry, cooking, and vacuuming.
- Transport and accompany individuals to appointments and community events.
- Perform and record physical assessments, including vital signs and medical histories.
- Exhibit acceptable work standards and promptly correct actions that do not meet agency standards.
- Complete required annual training by established deadlines.
- Follow Homemaker and Personal Care electronic and paper documentation requirements and ensure that Individual Service Plans and Behavior Support Plans are followed.
- Report UIR and MUI situations to the Site Leader according to provided guidelines.
- Keep and document records of body and general living conditions.
- Transport individuals in agency vehicles to community locations, including individuals who do not reside at the residential site.
- Perform other related tasks assigned by management.
- Adjust work hours as required for effective operation of the program.

The goal is to ensure IDD individuals have homes, environments, and opportunities to be integrated and included in their communities.
