Requirements
  • Graduate of an accredited school of nursing.
  • Certified as a nurse practitioner with current state licensure.
  • At least 1 year of previous professional nursing experience and 1-3 years of experience practicing as a primary care nurse practitioner.
  • Prior experience working in an electronic medical records system.
  • Advanced Cardiovascular Life Support and Basic Life Support certification as mandated by the state.
  • Knowledge of appropriate patient-care rationale.
  • Ability to handle emergency situations calmly and effectively.
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, and governmental regulations.
  • Ability to effectively present information and respond to questions from various groups of people.
  • Ability to define problems, collect data, establish facts, and draw valid conclusions.
  • Excellent written and verbal communication, teaching ability, and telephone skills.
  • Working knowledge of Google Suite, including Drive, Docs, Sheets, and Slides, and Microsoft Office, including Word, Excel, and PowerPoint, for real-time collaboration.
  • Ability to perform the physical requirements of the role, including handling and transferring patients, standing, walking, pushing, bending, kneeling, reaching, lifting and moving patients and medical equipment, and carrying up to 15 pounds.

Responsibilities
  • Practice medicine under physician supervision using sound medical judgment, adhering to ethical considerations, recognizing limitations, seeking consultation when advisable, and keeping current with technologies and industry standards.
  • Provide care for older adult patients with acute and chronic illness under physician supervision.
  • Remain accountable for patient care and outcomes, seek specialist consultation when needed, and stay involved in the detailed care of patients.
  • Engage with hospitalists whenever a patient is hospitalized.
  • Meet clinical obligations related to accrediting-agency compliance, Clinical Laboratory Improvement Amendments compliance, drug-prescription compliance, Health Insurance Portability and Accountability Act requirements, state and federal regulations, and safety regulations.
  • Assess, diagnose, treat, manage, educate, promote health, coordinate care, and document care for patients with acute and complex chronic health needs using evidence-based medicine and practice guidelines.
  • Follow internal policies and procedures.
  • Complete operational tasks according to departmental guidelines to ensure appropriate clinical processing and documentation.
  • Maintain complete and detailed electronic medical-records documentation of patients’ health-care plans and prognoses within 48 hours of each visit.
  • Provide patients with information to promote health, reduce risk factors, and prevent or manage disease or disability.
  • Analyze and interpret patient histories, symptoms, physical findings, and diagnostic information to develop appropriate diagnoses.
  • Interpret data and diagnostic results, including blood gases, radiology, and electrocardiograms, to determine an appropriate course of action in collaboration with the supervising physician.
  • Establish and maintain effective working relationships with patients, families, and team members.
  • Document continuing education in accordance with licensure and certification requirements.
  • Ensure patients are seen by a physician at every other visit.
  • Maintain patient confidentiality according to policy, procedure, and Health Insurance Portability and Accountability Act requirements.
  • Follow the proper chain of command for patient complaints.
  • Perform additional assigned duties.
