Requirements
  • High School Diploma or equivalent
  • Minimum 6 months to 1 year experience or internship in health care field or community outreach or community setting
  • Medical Assistant background highly desirable.
Responsibilities
  • Coordinating and managing outreach activities/classes relating to diabetes, cardiovascular disease and other chronic diseases
  • Co-facilitating classes in relation to the disease management program
  • Coordinating and setting up educational classes based upon the curriculum requested and/or required by AltaMed
  • Conducting series classes on adopted curriculum including but not limited to Healthy Living or Tomando Control
  • Following up on patient’s progress post classes as appropriate to determine and track effectiveness of program.
Desired Qualifications
  • Medical Assistant background highly desirable.
