- A Bachelor’s or Master’s Degree in Social Work is required.
- A state license is required.
- At least one year of supervised social work experience in a healthcare setting, working directly with patients and families, is required.
- Experience in a healthcare setting, preferably in a nursing home, hospice, home health, or hospital, is preferred.
- Ability to work with a variety of people in situations varying from community and nursing home planning is required.
- Ability to make decisions concerning procedures based on evaluations and interpretations of case histories is required.
- Assist with the pre-admission and admission process and help clients adjust to nursing home placement.
- Interview residents and families to obtain relevant past and present adjustment, behavior, and lifestyle information.
- Document progress and update plans of care at interdisciplinary care conferences.
- Arrange transportation, make appointments, and act as a liaison among departments, residents’ families, and outside agencies.
- Assist with handling and accepting placements through one-on-one counseling and group work.
- Assess potential and develop discharge plans when clients are admitted.
- Arrange post-discharge services and follow-up care.
- Assess residents’ emotional, financial, spiritual, educational, and social needs and ensure those needs are addressed by the facility or outside agencies.
- Act as a resident and family advocate and ensure residents understand and exercise their rights.
- Organize and conduct family council meetings and maintain meeting minutes.
- Maintain effective communication with residents, families, healthcare professionals, and facility staff.
- Experience in a healthcare setting, preferably in a nursing home, hospice, home health, or hospital.
