Requirements
  • A Bachelor's degree in Health Management, Business Administration, or another related field is required.
  • At least 1 year of experience in a similar management role is required.
  • Communication, organization, time management, and clinical skills are required.
  • The position requires working on-site on a rotating Monday-Friday and Tuesday-Saturday schedule.
  • The role requires maintaining confidentiality of consumer records and following documentation, procedures, and data-entry standards.
Responsibilities
  • Ensure all relevant documentation and procedures are followed by people entering the Drop-In Center, and keep information about Drop-In Center hours, locations, groups, and classes updated.
  • Complete, organize, and file periodic updates of member information in a timely manner.
  • Collect and enter data in specified locations as required by Community Reach Center standards, including data for the food pantry, meals, outings, events, and other services.
  • Facilitate or co-facilitate groups, discussions, or one-on-one services, and submit service data accurately and on time while maintaining confidentiality of consumer records.
  • Answer phones, connect callers with needed information or employees, and take and deliver phone messages.
  • Welcome consumers and visitors, direct them to community resources, navigate those resources until consumers can access them independently, and act as a liaison when necessary.
  • Enter new members into the Credible electronic health record system, update insurance information, and assist members with paperwork, general questions, and sign-in and sign-out procedures.
  • Keep the greeter table and bulletin board organized and updated daily with current and relevant information.
  • Cross-train other staff to improve the efficiency and effectiveness of service delivery.
  • Provide navigation and support services through integrated behavioral healthcare, using case management and care coordination to connect consumers with community resources through referrals, linkage, and advocacy.
