Requirements
  • A Bachelor's degree in Social Work, Education, Psychology, or a related field is required.
  • At least three years of professional experience in a health care, governmental, educational, preferably K-12, or related setting is required.
  • Proficiency in electronic systems including Google Suite, Trayt, Slack, Athena or Epic, and Calendly is required.
  • Relevant education and experience may be substituted as appropriate.
  • A criminal history background check is required for finalists.
  • Applicants must provide a resume or CV, three work references with contact information including at least one supervisor reference, and a letter of interest.
  • Successful hires must complete federal Employment Eligibility Verification and provide acceptable documents proving identity and authorization to work in the United States.
Responsibilities
  • Work in an interdisciplinary model with pediatric psychiatrists, psychologists, licensed mental health professionals, and operational staff to support students and families receiving care.
  • Assist with navigating treatment recommendations and assemble clinical background and referral information.
  • Schedule, cancel, and reschedule patient appointments.
  • Complete accurate and timely clinical-record documentation in accordance with professional ethics and federal and state regulations.
  • Identify and, as clinically indicated, connect students and families to appropriate community resources for long-term care.
  • Provide ongoing support to ensure effective transitions and minimize barriers to care.
  • Coordinate with professionals, schools, and families to enhance satisfaction and service delivery.
  • Serve as a liaison for records management and general inquiries related to program-specific services.
  • Oversee internal and external communication regarding clinical services, discharge planning, and school collaboration.
  • Support dissemination of referral information and document interactions in the electronic medical record.
  • Collaborate with designated program staff in using the identified community resource database and assist in solving complex cases.
  • Develop and deliver training for new hires.
  • Participate in special projects as assigned.
  • Participate in interdisciplinary team meetings, case reviews, and planning sessions to support collaborative care.
  • Assist with data collection and reporting for program evaluation, patient outcomes, and referral trends.
  • Contribute to quality improvement initiatives by identifying service gaps and recommending enhancements to care coordination processes.
  • Provide backup support for other care coordination staff during absences or periods of high service demand.
  • Perform related duties as required.
  • Track referral outcomes and care-plan adherence, use data to improve service delivery, ensure timely follow-up and documentation, maintain organized referral databases, coordinate multiple cases simultaneously, identify barriers to care, develop solutions, and collaborate to resolve service gaps.
Desired Qualifications
  • A Master's degree in Social Work, Psychology, Education, or a related field is preferred.
  • Knowledge of managed care, medical terminology, patient referral processes, claims, and Current Procedural Terminology coding is preferred.
  • Experience working with pediatric behavioral and mental health patient populations is preferred.
  • Knowledge of educational practices supporting mental health and social-emotional learning in K-12 settings is preferred.
  • Experience developing, refining, and evaluating patient care plans or student education plans is preferred.
  • Bilingual English/Spanish language proficiency is preferred.
