Requirements
  • A minimum high school diploma or equivalent is required.
  • Experience providing advocacy services to people who are mentally ill and/or homeless is required.
  • Excellent verbal, written, and interpersonal communication skills are required.
  • The ability to work reliably and independently while maintaining confidentiality is required.
Responsibilities
  • Serve as a role model through lived experience and educate individuals about self-help techniques and self-help group processes.
  • Teach effective coping strategies based on personal experience and teach wellness management skills.
  • Assist participants in clarifying rehabilitation and recovery goals and developing community support systems and networks.
  • Facilitate seamless transitions between levels of care by addressing participants’ preparatory needs during recovery stages.
  • Support discharge planning, development of daily living skills, and coping mechanisms through team and individual work in the community.
  • Engage participants in peer-based community services to identify and overcome barriers to community integration.
  • Use peer relationships to foster connection, trust, understanding, and validation with community providers and supports.
  • Work as part of a multidisciplinary team.
  • Participate in transition planning, including transition needs assessments of community and natural supports.
  • Conduct individual sessions emphasizing prevention, independent community living, and optimum mental and emotional health.
  • Help participants address family and social relationships, stress and symptom management, activities of daily living, medication management, and housing readiness.
  • Work with participants and housing providers to resolve clinical issues affecting participants’ ability to obtain, manage, and retain housing.
  • Assist and instruct participants in personal hygiene, grooming and laundry, nutrition and meal preparation, budgeting, socialization, and maintaining a safe living environment.
  • Foster and maintain collaborative relationships with community providers and network members to connect or reconnect participants to services and supports.
  • Provide community navigation, including accompanying participants to behavioral health and medical appointments, travel training, reengagement in community care, and referrals to services.
  • Identify and address potential service barriers affecting participants’ ability to obtain and maintain services.
  • Advocate with and for participants to achieve personal goals and address barriers such as fear, stigma, and communication disparities.
  • Provide on-call after-hours crisis intervention services, including respite referrals and other diversion and stabilization services.
  • Attend and participate in team meetings and supervisory sessions.
  • Develop person-centered treatment plans to help participants achieve their goals.
  • Monitor and record participant progress and modify plans according to participant needs and preferences.
  • Perform needs assessments for ongoing community integration and support.
  • Document participant information accurately and promptly within the health information technology platform.
  • Complete required in-service training and staff development.
  • Perform other related duties as assigned.
Desired Qualifications
  • New York State Certified Peer Specialist certification is preferred.
  • Past or current participation in behavioral health, substance-use, or homeless services is preferred.
  • Strong organizational skills and timely follow-up to manage complex tasks are preferred.
  • Proficiency in Microsoft Word and Microsoft Excel is preferred.
  • Bilingual ability in any language is a plus but is not required.
