Requirements
  • Valid and active RN License in the state of Oregon
  • Minimum of 2 years of professional or management experience in a health-related field or program
  • Ability to provide support to patients in Portland and surrounding areas
  • 2+ years of experience in operations management in healthcare, business, or customer service
  • Experience in a startup environment
  • Experience using a CRM
  • Proficient in Microsoft Office and Google Suite

Responsibilities
  • Assist the Manager of Operations in managing day-to-day operations in Oregon
  • Assist in building up the team to achieve growth goals in Oregon
  • Coordinate immediate care interventions and communicate effectively with the care team
  • Coach, develop, and manage a team of patient navigators
  • Maintain strict compliance with healthcare regulations, company policies, and industry standards
  • Assist with documenting and developing best practices and training material
  • Implement strategy for patients and caregivers, leveraging technology to streamline processes and improve quality of care
  • Participate in hiring, selection, and onboarding/training process for new employees
  • Oversee and manage day-to-day clinical operations in compliance with Oregon Health Authority regulations
  • Engage with patients over the phone to address inquiries and provide support
  • Collaborate with healthcare professionals, caregivers, and patients for a patient-centered approach to care
  • Conduct and document comprehensive health assessments and develop individualized care plans
  • Administer and oversee medication management in accordance with regulations
  • Train, supervise, and evaluate caregivers and healthcare staff
  • Respond to health emergencies and coordinate with healthcare professionals
  • Provide education to clients and families on managing health conditions
  • Maintain accurate and confidential records of client care
  • Review and adjust client care plans based on health evaluations
  • Enforce processes and policies to support high standards of care
  • Identify and work with development and technology teams for software adjustments
  • Build a positive work environment and culture consistent with company values
  • Ensure all policies and procedures demonstrate commitment to high ethical standards
