Requirements
  • The candidate must hold a Licensed Practical Nurse license issued by the Wisconsin State Board of Nursing or the state where the Rogers clinical facility is located, and the license must be in good standing.
  • The candidate must obtain American Heart Association Healthcare Provider CPR certification or American Red Cross Professional Rescue certification within thirty days of hire and complete annual recertification.
  • The candidate must complete formal training in management of the aggressive patient within sixty days of hire and complete annual recertification.
  • The candidate must comply with applicable nursing practice standards, hospital policies and procedures, infection control requirements, employee health policies, Joint Commission requirements, and Wisconsin regulatory requirements.
Responsibilities
  • Implement patient care for an assigned patient load under the direction of a registered nurse.
  • Observe, document, and monitor patients’ psychiatric and physical changes and responses to treatment, including admission data collection, discharge transcription, medication reconciliation, and medication administration.
  • Identify potential patient-care problems, abrupt changes, or impending instability and intervene appropriately to prevent adverse outcomes.
  • Use de-escalation techniques, including a quiet room, locked seclusion, or restraints, when appropriate.
  • Re-evaluate patients’ safety levels and identify alcohol withdrawal syndrome and significant cardiac or respiratory symptoms requiring immediate medical intervention.
  • Implement established care plans and unit standards of care while incorporating patients’ age-specific and cultural needs.
  • Participate in nursing assessments, obtain and process physician orders, complete medication-reconciliation orders, and create treatment plans under appropriate clinical supervision.
  • Prepare tube-feeding dosages according to physician orders and administer bolus tube feedings.
  • Administer medications safely, accurately transcribe and implement physician medication orders after registered-nurse approval, monitor medication effects and adverse reactions, document medication responses, educate patients about medications, obtain medication consent, and maintain current medication knowledge.
  • Help patients follow program schedules and address their hygiene, physical-environment, and personal-belongings needs.
  • Monitor patients’ locations in assigned areas and accompany patients to meals while monitoring behavior and recording food intake.
  • Explain patient rights, obtain required signatures and consents, assist with complaints and grievances, and protect patient privacy and confidentiality.
  • Assist with patient orientation, including unit-safety procedures, gown and contraband searches, and provision of pertinent unit information.
  • Protect patients, visitors, and staff from environmental hazards by following safety and infection-control standards.
  • Participate in continuing education, inservice programs, seminars, hospital committees, performance-improvement meetings, and team projects.
  • Develop and review professional-growth goals with the Clinical Services manager and seek projects or extra tasks based on unit needs.
  • Communicate department goals, maintain department policies and procedures, communicate the function backlog, and educate new staff and student interns regarding applicable regulations and requirements.
  • Participate in education regarding Nursing department regulations and apply Joint Commission and other regulatory standards.
  • Demonstrate punctuality, preparedness, organization, timely task completion, professional communication, and constructive issue resolution.
  • Communicate concerns and proposed solutions, maintain a professional image, and comply with hospital policies and programs.
