Requirements
  • A bachelor's degree in Business, Marketing, Healthcare Administration, Public Health, or a related field, or an equivalent combination of education and relevant experience.
  • At least one year of experience in healthcare sales, business development, provider relations, community outreach, enrollment, or a similar relationship-focused role.
  • Ability to develop and maintain trusted relationships with healthcare providers, community organizations, referral partners, and other external stakeholders to support organizational growth.
  • Experience identifying, cultivating, and managing referral sources, community partnerships, or business development opportunities that contribute to enrollment growth.
  • Experience working with older adults, frail elderly populations, and caregivers.
  • Strong organizational skills with the ability to manage multiple priorities, maintain accurate records, and ensure timely follow-through.
  • Strong communication, presentation, and collaboration skills, including the ability to educate healthcare providers, community partners, prospective participants, caregivers, and cross-functional teams about the value of the PACE program.
  • Ability to work independently while collaborating across teams to achieve shared outcomes.
  • Experience using Microsoft Office and the ability to learn CRM, referral management, and participant relationship management systems.
  • Proof of a valid driver's license, reliable personal transportation, and automobile insurance meeting applicable state requirements, with the ability to travel throughout the service area.
  • Active CPR/BLS certification or the ability to obtain certification within 30 days of hire.
  • Ability to meet applicable participant-facing health requirements, including TB screening and required immunizations.
  • Ability to adapt quickly and contribute to continuous improvement in a fast-paced, evolving environment.
  • Ability to seek feedback, engage in healthy debate, and use data and curiosity to inform decisions.
  • Ability to act with integrity and ownership, consider broader organizational impact, and follow through to deliver results.
Responsibilities
  • Build trusted relationships with healthcare providers, community organizations, senior living communities, post-acute providers, and other referral partners to generate qualified referrals and support enrollment growth.
  • Develop and execute outreach strategies that increase awareness of the PACE program and strengthen community partnerships.
  • Represent Habitat Health through community presentations, networking events, educational sessions, health fairs, and other outreach initiatives.
  • Drive the enrollment process as the case manager for prospective participants by reviewing, processing, and updating applications and required documentation, distributing enrollment materials, and managing communications.
  • Identify opportunities to expand referral sources, strengthen community partnerships, and improve outreach strategies using market insights and performance data.
  • Maintain accurate referral, outreach, and relationship activity in CRM and other organizational systems in a timely manner.
  • Monitor outreach activities and performance metrics, use data to prioritize work, and contribute to continuous improvement.
  • Ensure outreach and marketing activities comply with organizational policies and federal and state regulatory requirements governing PACE marketing practices.
  • Participate in evening and weekend community events as needed to support organizational growth objectives.
  • Conduct telephonic, home, and in-center evaluations of prospective participants for potential care eligibility.
  • Collaborate with Center leadership to develop retention strategies.
  • Communicate with participants, caregivers, team members, and other stakeholders in person, by phone, and through electronic systems.
  • Operate standard office equipment and clinical tools, including computers, phones, medical devices, and documentation systems.
  • Travel between work sites and community locations as needed.
  • Follow safety procedures and infection-control protocols and use personal protective equipment when required.
  • Perform duties involving bending, reaching, lifting, or assisting participants, depending on role responsibilities.
  • Lift, carry, push, or pull supplies, equipment, or materials typically weighing up to 25–50 pounds with assistance or mechanical aids, depending on job duties.
Desired Qualifications
  • Experience working with Medicare, Medicaid, state and county eligibility programs, or other healthcare eligibility and referral processes.
  • Experience supporting outreach, business development, provider relations, or participant engagement within PACE, Medicare, Medicaid, managed care, or other value-based healthcare environments.
  • Knowledge of PACE, the Centers for Medicare & Medicaid Services, and applicable federal and state regulatory requirements related to outreach, participant eligibility, enrollment, and care coordination.
  • Experience using CRM, referral management, or participant relationship management systems.
  • Bilingual ability in Spanish, Mandarin, or Cantonese.

Habitat Health holds ribbon cutting for South Los Angeles PACE Center. February 27, 2026 Habitat Health, in collaboration with Kaiser Permanente, officially celebrated the opening of its South Los Angeles Program of All-Inclusive Care for the Elderly (PACE) Center today with a ribbon-cutting ceremony attended by community leaders and healthcare partners. The event marked a significant milestone in expanding access to coordinated, whole-person care for older adults age 55 and over in one of Los Angeles County's most medically underserved regions. "Today is more than the opening of a building. It marks a new chapter in how we care for older adults in South Los Angeles," said Sophia Guel-Valenzuela, Executive Director of Habitat Health South Los Angeles. "For too long, many older adults and their caregivers in this community have faced barriers to coordinated outpatient care. This center was built with those realities in mind. We are honored to be another PACE access point providing comprehensive, culturally responsive care directly for the neighborhoods that need it most." South Los Angeles experiences disproportionately high rates of chronic conditions such as diabetes, asthma, heart disease, and hypertension. Adults in the region face significantly higher rates of preventable hospitalizations and emergency department visits compared to other parts of the county. In fact, diabetes-related hospitalizations in South Los Angeles are 2.5 times higher than in West Los Angeles, highlighting serious gaps in preventive and coordinated care. "Opening the doors to this Habitat Health PACE center means opening doors to longevity, connection, and health for the community of South Los Angeles. Habitat Health's patients will have access to Kaiser Permanente's model, which is grounded in providing the right care, at the right place, at the right time - whether that's inside of our clinic walls or beyond. We're proud to play a key role in keeping this community healthy, independent, and active," said Dr. Bechara Choucair, chief health officer, executive vice president, Kaiser Permanente, and a Habitat Health board member. Habitat Health is both a PACE health plan and a direct care provider, offering integrated medical and social services under one roof. Services include primary and specialty care coordination, social services, day center activities, transportation, physical therapy, prescription management, nutrition counseling, home care coordination, dental services, and access to diagnostic and specialty services. Through its collaboration with Kaiser Permanente, participants also receive coordinated specialty and hospital-based care through Kaiser Permanente's regional network. For individuals who qualify for both Medicare and Medi-Cal, PACE services are typically available at no cost. The Compton center is now accepting referrals and applications.
