  • Previous experience with population health, case management, quality performance measures, and value-based care reimbursement models is required.
Responsibilities
  • Design and direct the value-based care program in compliance with regulatory requirements.
  • Develop and refine value-based care models tailored to organizational goals.
  • Monitor, interpret, and implement regulatory changes impacting value-based care.
  • Coordinate with the compliance team and resources to ensure program alignment with current laws and regulations.
  • Lead regulatory reporting activities and prepare and maintain documentation.
  • Create and enforce compliance protocols for all facets of the value-based care program and conduct regular compliance audits to address identified gaps.
  • Collaborate with the quality improvement team to maintain accreditation and regulatory standards.
  • Provide compliance training and education to staff.
  • Lead value-based care initiatives and facilitate their integration into operational workflows.
  • Oversee daily operations of value-based care programs, ensuring services are responsive, organized, and focused on improving health outcomes.
  • Assess workflows and recommend improvements for enhanced care delivery and operational efficiency.
  • Establish process improvement initiatives and track operational outcomes.
  • Mentor and collaborate with operational leaders and teams to ensure consistent execution of value-based care program objectives.
  • Design and implement revenue strategies for value-based care initiatives to achieve financial sustainability.
  • Analyze reimbursement trends and recommend program adjustments to maximize returns.
  • Collaborate with finance and analytics teams to monitor program financial performance.
  • Create business plans and reports for leadership and stakeholders.
  • Collaborate with clinical, administrative, and information technology teams to ensure integrated care delivery.
  • Engage with external stakeholders, including payers, regulators, and community partners, and identify opportunities to advance program objectives.
  • Facilitate meetings and communications to drive awareness and adoption of value-based practices.
  • Gather and act on feedback from patients, families, and staff to promote continuous improvement.
  • Provide overall management and direction to departmental staff.
  • Oversee the development and maintenance of the departmental budget, monitor budgetary compliance, and address problems as needed.
  • Delegate departmental duties and responsibilities and participate in high-priority special projects and activities.
  • Represent the department at senior management meetings.
  • Prepare departmental company policies and procedures and communicate senior management directives.
  • Monitor departmental performance using company performance standards and address issues as needed.
  • Direct departmental performance and provide specific instructions on completing tasks and responsibilities.
  • Prepare and conduct performance appraisals for immediate staff.
  • Conduct hiring, disciplinary, and termination procedures.
