  • Current, unencumbered license to practice as a nurse in Pennsylvania and will remain in good standing with the State Board of Nursing

Responsibilities
  • Provide direct nursing care to residents and supervise the day-to-day nursing activities performed by certified nursing assistants under the supervision of the Director of Nursing and Assistant Director of Nursing to ensure compliance with current state, federal, and local standards
  • Participate in the development, maintenance, and implementation of the facility’s quality assurance program for the nursing service department and participate in facility surveys (inspections) by government agencies as requested
  • Admit, transfer, and discharge residents as required; greet newly admitted residents upon admission; escort them to their rooms as necessary; participate in orientation of new residents and family members; make rounds with physicians as necessary; requisition and arrange for diagnostic and therapeutic services; consult with the resident’s physician in providing the resident’s care and treatment; review the resident’s chart for treatments, medications, orders, diets, etc.; coordinate with physicians as necessary
  • Receive/give the nursing report upon reporting in and ending shift duty hours
  • Charting and documentation: complete and file required recordkeeping forms/charts upon admission, transfer, and/or discharge; ensure that nurses’ notes reflect that the care plan is being followed; review resident care plans for goals and revisions; ensure CNAs are aware of care plans
  • Ensure that assigned CNAs are aware of the resident care plans and refer to the care plan prior to administering daily care
  • Review the resident’s discharge plan and assist in planning the nursing services portion of the discharge plan; interpret department policies and procedures to personnel, residents, visitors, and government agencies as required
  • Complete accident/incident reports and submit to Director; chart all accident/incident reports involving residents
  • Inform family members of the death of the resident and coordinate post mortem procedures as requested
  • Write resident charge slips and forward to the Business Office; maintain the Daily Census Report and submit to the Business Office as required
  • Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc.
  • Ensure that personnel providing direct care to residents are providing such care in accordance with the resident’s care plan and wishes
  • Ensure that residents who are unable to call for help are checked frequently; meet with residents and/or family members as necessary
  • Report problem areas to the Nurse Supervisor; admit, transfer and discharge residents as necessary; assist in arranging transportation for discharged residents
  • Chart nurses’ notes in an informative and descriptive manner; perform routine charting duties; sign and date all entries in the resident’s medical record; administer medications as ordered following verification steps; ensure narcotic records are accurate and report discrepancies; dispose of drugs and narcotics as required
  • Implement and maintain nursing objectives and standards; periodic checks to ensure prescribed treatments are properly administered by CNAs and evaluate residents’ status
  • Record diet orders and communicate with the Food Services Department; report discrepancies in orders; complete transfer forms as required
  • Maintain an adequate supply of floor stock medications, supplies, and equipment; order medications and supplies as necessary; ensure safe operation of nursing service equipment
  • Assist the Resident Assessment/Care Plan Coordinator in planning, scheduling, and revising the Minimum Data Set (MDS) including RAPs and Triggers
  • Participate in staff development activities including in-service training, orientation, and continuing education; participate in infection control and safety programs; ensure compliance with dress code and policies
  • Supervisory responsibilities as a Licensed Practical Nurse/Licensed Vocational Nurse include delegating administrative authority and accountability for duties
  • Make daily rounds to ensure nursing service personnel perform duties to established standards; report issues to the Nurse Supervisor and maintain a positive unit environment
  • Ensure departmental policies and procedures are followed; maintain good working relationships with other departments
  • Create and maintain a warm, personal environment on the unit; provide leadership to nursing personnel
  • Report occupational exposures and incidents; report fraud and ensure proper computer security practices; participate in infection control and safety programs
  • Inform nursing personnel of new admissions and prepare rooms; notify physician in case of accidents or changes in condition; carry out restorative programs; supervise and participate in care
  • Inspect nursing and treatment areas daily to ensure cleanliness and safety; perform procedures such as catheterization, tube feedings, dressing changes, and other necessary nursing tasks
  • Obtain specimens for laboratory tests and monitor vital signs; monitor seriously ill residents
  • Participate in staff development, training, and continuing education; ensure compliance with safety, infection control, and nursing standards
