Requirements
  • A Certified Medical Assistant or Licensed Practical Nurse license is required.
  • A valid driver's license and access to a vehicle are required.
  • At least two years of management experience in a healthcare setting is required.
  • Knowledge of principles and processes for providing customer and patient services, including customer needs assessment, meeting quality standards for services, and evaluating customer satisfaction is required.
  • Knowledge of business and management principles involved in strategic planning, resource allocation, human resources modeling, leadership technique, production methods, and coordination of people and resources is required.
  • Proficiency in the structure and content of the English language, including the meaning and spelling of words, rules of composition, and grammar, is required.
  • Understanding of principles and methods for curriculum and training design, teaching and instruction for individuals and groups, and measuring training effects is required.
  • Familiarity with principles and procedures for personnel management is required.
  • Travel to facilities and between clinics is required.
Responsibilities
  • Maintain a consistent embedded presence at assigned clinics and serve as a practicing Care Coordinator for Ennoble Care's patient panel where applicable.
  • Execute core daily Care Coordinator responsibilities, including prescription management, laboratory and radiology orders, referral coordination, pre-visit check-ins, missed-treatment outreach, and post-discharge follow-up.
  • Coordinate between facility staff and Ennoble Care's services, including house calls, hospice, and ancillary programs.
  • Conduct proactive rounds with patients and clinic staff, respond to acute patient needs, and escalate to providers as appropriate.
  • Meet the daily task-minute goal of 400 minutes unless engaged in approved training or onboarding activities.
  • Support facilitated telehealth and remote patient monitoring programs within assigned facilities.
  • Directly oversee a team of 8–12 Embedded Care Coordinators across multiple facilities or markets.
  • Provide day-to-day guidance, coaching, and support to ensure Care Coordinators execute responsibilities consistently and to standard.
  • Serve as the primary escalation and problem-solving resource for the Embedded Care Coordinator team.
  • Monitor individual and team performance against key performance indicators, address performance gaps, and escalate issues as needed.
  • Coordinate with providers to ensure they have everything needed from the Care Coordinator team.
  • Partner with leadership on staffing decisions, performance management, and team development.
  • Set and model standards for documentation quality, reliability, communication, and patient engagement.
  • Backfill for Embedded Care Coordinators as needed to maintain care continuity.
  • Travel to facilities to oversee Care Coordinators and assist as needed.
  • Support onboarding and training of new Embedded Care Coordinators, including completion of credentialing and training modules.
  • Support new Embedded Care Coordinators through their ramp period by setting performance expectations and conducting regular check-ins.
  • Build and maintain working relationships with clinic staff to support day-to-day coordination.
  • Represent the Care Coordinator team professionally in joint meetings.
  • Monitor and report clinical and operational metrics weekly, and track and escalate metrics trending off target.
  • Ensure accurate, up-to-date patient documentation in OA.
  • Flag operational issues, workflow gaps, and patient concerns promptly and propose solutions.
  • Help refine and update Care Coordinator tools, checklists, and standard operating procedures as the program evolves.

  • Familiarity with facility-based operations and workflows.
  • Experience onboarding or training colleagues in care coordination workflows.
